摘　　要：目的 探讨血液肝癌细胞相关甲胎蛋白（AFP） mRNA阳性与肝癌患者术后肿瘤复发或转移的关系.方法 应用巢式RT-PCR检测62例原发性肝癌患者术前1天周围静脉血液中AFPmRNA,术后随访3年.结果 24例术前血液检测AFPmRNA阳性,38例患者术前AFPmRNA呈阴性.术前血AFPmRNA阳性的患者术后1、2、3年肿瘤复发率分别为58.3％ （14/24）、75.0％（18/24）、100.0％ （24/24）;而术前血AFPmRNA阴性的患者术后1、2、3年肿瘤复发率则分别为23.6％ （10/38）、42.1％（16/38）、68.4％（26/38）,两者比较复发率差异具有统计学意义（P＜0.01）.术前血AFPmRNA阳性的患者术后3年有8.3％ （2/24）发生肺转移;而阴性患者术后3年内无一发生肺或其他远处转移（0.0％,0/38;P＜0.01）.结论 术前血液AFPmRNA阳性的肝癌患者较AFPmRNA阴性的肝癌患者术后更容易产生复发或远处转移.Objective To study the correlation between positive peripheral blood hepatocellular carcinoma （HCC） cells-associated AFPmRNA and HCC recurrence / distant metastasis after HCC resection.Methods We studied blood AFPmRNA in 62 HCC patients using nested RT-PCR 1 day before hepatectomy,and followed these patients up for 1 to 3 years after surgery.Results 24 HCC patients were tested AFPmRNA positive before surgery,while 38 HCC patients were tested negative.Of the 24 patients who were tested positive for AFPmRNA,14 relapsed （14/24,58.3％） within the first year of surgery,4 in the second year （18/24,75.0％）,the remaining 6 in the third year （24/24,100％）.For the 38 patients who were tested AFPmRNA negative,10 patients （10/38,26.3％） relapsed within the first year,6 in the second year （16/38,42.1％）,and 10 in the third year （26/38,68.4％,P ＜ 0.01）.There was significant difference between patients who were tested AFPmRNA positive and AFPmRNA negative in relation to tumor recurrence （P ＜ 0.01）.Among 24 patients who were tested AFPmRNA positive,2 patients developed lung metastasis in the third year （2/24,8.3％）.There was no patient who developed lung metastasis or distal metastasis in the third year in the patients who were tested AFPmRNA negative （0/38,0.0％,P ＜ 0.01）.Conclutions HCC patients who were tested AFPmRNA positive before operation were more prone to develop HCC recurrence or distal metastasis when compared with those patients who were tested AFPmRNA negative.
