枫香脂联合左旋甲状腺素片对分化型甲状腺癌患者术后促甲状腺素抑制治疗的疗效观察

摘　　要：目的 观察应用枫香脂联合左旋甲状腺素对分化型甲状腺癌患者术后促甲状腺激素、总体甲状腺激素水平及游离甲状腺素水平的影响.方法 分化型甲状腺癌患者采用简单随机化方法分成两组,单纯用药组（n=55,口服左旋甲状腺素片）和联合用药组（n=57,口服枫香脂联合左旋甲状腺素）,比较用药前后两组患者血清中促甲状腺素（TSH）、总甲状腺素（T4、T3）和游离甲状腺素（FT3、FT4）的变化.结果 联合用药组与单纯用药组相比,两组之间用药前TSH、T4、T3、H4、FT3均没有明显差异,用药后联合用药组的TSH水平明显低于单纯用药组[（0.44±0.10） μIU/ml比（0.94±0.25） μIU/ml,F=12.34,P＜0.05]而FI3升高程度明显高于单纯用药组[（1.08±0.39） pmol/ml比（0.66±0.36） pmol/ml,F=6.9,P＜0.05],差异具有统计学意义.结论 枫香脂联合左旋甲状腺素可以在不增加左旋甲状腺素剂量的情况下,提高分化型甲状腺癌患者血清中游离甲状腺素FT3的水平,增强TSH抑制治疗效果.Objective To investigate the therapeutic efficacy of postoperative thryrotropin （thyroid stimulating hormone,TSH） suppressive therapy of liquidambaris resina combined levothyroxine on differential thyroid carcinoma （DTC）.Methods Postoperative DTC patients were randomly divided into two groups.57 patients in group A treated with liquidambaris resina combined with levothroxine,while in group B 55 cases treated with levothyroxine alone.Serum level of TSH,T3,T4,FT3 （Free T3）,Free T4 （FT4） were determined before and after TSH-suppressive therapy.Results There were no statistical difference in the level of TSH,T4,T3,FT4 and FT3 between the two groups before TSH-suppressive therapy started.But there were statistical difference in the blood concentration of TSH and FT3 between the two groups.The level of TSH in group A was significantly lower than in group B [（0.44 ± 0.10） μIU/ml vs.（0.94 ±0.25） μIU/ml,F =12.34,P ＜ 0.05],and the level of FT3 in group A was significantly higher than in group B [（1.08 ±0.39） pmol/ml vs.（0.66 ±0.36） pmol/ml,F =6.9,P ＜0.05].Conclusions TSH-suppressive therapy with liquidambaris resina combined levothyroxine increases the level of FT3 compared with levothyroxin therapy alone,combined therapy has more powerful TSH-suppressive efficacy in DTC patients.
