丝裂霉素C在难治性青光眼中疗效及影响

摘　　要：目的 观察丝裂霉素C在难治性青光眼患者中的疗效及其对血清维生素、白介素的影响研究.方法 临床病例对照研究.将2009年1月至2013年2月在上海仁济医院眼科收诊的难治性青光眼患者采用数字表法,随机分成两组,对照组（84只眼）和观察组（89只眼）.所有患者均行复合式小梁切除术,观察组患者在此基础上加用丝裂霉素C治疗.比较两组间疗效及治疗前后眼压、视力差异;比较治疗前后两组间血清叶酸（FA）、维生素B12（VB12）、维生素B6及白介素-2（IL-2）、白介素-6（IL-6）水平差异.结果 观察组手术成功率为80.89％ （68/84）显著高于对照组的60.71％（54/89）,术后观察组眼压显著低于对照组,视力显著高于对照组,差异有统计学意义（P＜0.05）.手术未成功组患者血清FA、、VB12、IL-2、IL-6显著低于正常对照组和手术成功组,差异有统计学意义（P＜0.05）.术后第3个月观察组患者血清FA、VB12、IL-2、IL-6显著高于对照组,差异有统计学意义（P＜0.05）.结论 在复合式小梁切除术中加载丝裂霉素C能有效提高难治性青光眼的治疗成功率,改善血清维生素、白介素相关指标.Objective To investigate the efficacy of Mitomycin C（MMC）for patient with refractory glaucoma and its influence on the serum vitamin and interleukin.Methods Patient with refractory glaucoma from January 2009 to January 2013 were randomly divided into control group（compound trabeculectomy treatment group）80 cases（84 eyes）and observation group（MMC combined with compound trabeculectomy treatment group） 80 cases（89 eyes）,then the difference in efficacy between the two groups were counted,and the intraocular pressure,visual acuity and the levels of FA,VB12,VB6,IL-2 and IL-6 of two groups before and after the treatment were compared.Results The surgical success rate of observation group was 80.89%（68/84）which was significantly higher than the control group of 60.71%（54/89）; after the surgery,the intraocular pressure of observation group was all lower than control group,and the visual acuity was better than control group,there were all significant differences（P 〈 0.05）.The levels of FA,VB12,IL-2 and IL-6 of observation group at 3rd month after the treatment were all higher than those indexes of control group,there were all significant differences （P 〈0.05）.Conclusions Combined with MMC can effectively improve surgical success rate of refractory glaucoma,and improve the indicators of serum vitamin,interleukin.
