大肠癌术后依据TopoI检测个体化治疗与传统治疗的疗效比较

摘　　要：目的:研究大肠癌中检测TopoI表达来选择术后个体化辅助化疗方案的临床意义。方法:将84例大肠癌患者根治术后的石蜡标本行免疫组化检测TopoI的表达情况,根据蛋白检测结果选择患者个体化的化疗方案。未行TopoI检测的同期对照组按传统方法选择化疗方案,对比分析两组患者的无进展生存时间(progress free survival,PFS)、中位生存期(median survival time,MST)和疾病控制率(disease control rate,DCR)。结果:大肠癌组织中TopoI的表达与性别、肿瘤大小、分化程度、肿瘤分期无关,而与年龄有关(P<0.05)。依据TopoI蛋白检测选择化疗方案组中位PFS为7.8个月;对照组中位PFS为7.2个月,两组PFS比较差异有统计学意义[(7.55±1.53)个月vs(6.93±1.62)个月,P<0.05]。实验组的中位生存期为17.2个月,对照组为13.6个月。而且IV期患者实验组疾病控制率显著升高(80.8%vs 52.6%,P<0.05)。结论:通过检测大肠癌术后TopoI的表达来选择个体化的治疗方案,具有重要的临床指导价值。Objective：To explore the clinical significance of individualized postoperative treatment based on TopoⅠ detection of colorectal cancer.Methods：The TopoⅠ expression was detected by immunohistochemistry on paraffin sections of 84 patients with resection of colorectal cancer,and then select individualized chemotherapy regimens according to the detection result.The control group without TopoⅠ detection select the chemotherapy regimens by traditional method.The PFS,MST and DCR were compared and analyzed between two groups.Results：The TopoⅠ expression was correlated with age in colorectal cancer（P 〈 0.05）.There was no statistically correlation between TopoⅠ expression and sex,tumor size,differentiated degree and clinical TNM staging.The median PFS of individualized postoperative treatment group based on gene detection was 7.8 months,and that was 7.2 months in control group.There was significant difference of PFS between two groups（7.55 ± 1.53 vs 6.93 ± 1.62 months,P 〈0.05）.The median survival time of experimental group and control group were 17.2 months and 13.6 months respectively.And also the DCR of the Ⅳ patients was significantly higher in experimental group （80.8% vs 52.6%,P 〈0.05）.Conclusion：It has important clinical guidance value to select individualized postoperative chemotherapy regimens according to TopoⅠ detection result in colorectal cancer.
