摘 要:目的观察线粒体细胞色素C氧化酶Ⅰ(cytochrome coxidaseⅠ,COXⅠ)和线粒体细胞色素C氧化酶Ⅲ(cytochrome coxidaseⅢ,COXⅢ)在正常结直肠黏膜、结直肠高级别上皮内瘤变(原位癌)和结直肠癌组织中的表达,探讨COXⅠ和COXⅢ在结直肠癌早期诊断中的作用以及二者与结直肠癌患者预后的关系。方法采用免疫组化SP法检测20例结直肠正常黏膜、20例结直肠高级别上皮内瘤变(原位癌)和80例结直肠癌组织中COXⅠ和COXⅢ的表达,并分析二者表达与结直肠癌临床病理特征及患者预后的关系。结果 COXⅠ和COXⅢ在正常结直肠黏膜中的阳性率分别为75.0%(15/20)和70.0%(14/20),在结直肠高级别上皮内瘤变(原位癌)组织中的阳性率为35.0%(7/20)和25.0%(5/20),在结直肠癌组织中的阳性率为36.2%(29/80)和26.2%(21/80);COXⅠ和COXⅢ在正常结直肠黏膜和结直肠高级别上皮内瘤变(原位癌)中的阳性率差异有统计学意义(χ2=6.465,P=0.011;χ2=8.120,P=0.004),COXⅠ和COXⅢ在正常结直肠黏膜和结直肠癌组织中的阳性率差异有统计学意义(χ2=9.75,P=0.002;χ2=13.46,P<0.001)。COXⅠ的阳性率与结直肠癌组织学分级、临床分期相关;组织学分级越低,COXⅠ阳性率越高,Ⅲ+Ⅳ结直肠癌组中COXⅠ阳性率低于Ⅰ+Ⅱ组。COXⅢ的阳性率与结直肠癌临床分期相关,Ⅲ+Ⅳ期结直肠癌组中COXⅢ阳性率低于Ⅰ+Ⅱ期组。Kaplan-Meier生存分析表明COXⅠ和COXⅢ表达与结直肠癌患者预后相关(P=0.008,P=0.017)。结论 COXⅠ和COXⅢ高表达与结直肠癌组织学分级、临床分期、预后密切相关,联合检测二者表达可作为结直肠癌早期诊断及判断其生物学行为的重要指标。Purpose To investigate the expression levels of cytochrome coxidase I ( COX I ) and cytochrome c oxidase Ⅲ ( COX Ⅲ) in colorectal cancer and their relationships with the occurrence, development, and prognosis of colorectal cancer. Methods The im- munohistochemical SP method was used to detect the expression of the COX I and COXⅢ proteins in eolorectal tissue. The correlations of COX I and COX Ill with clinicopathologic features were analyzed in colorectal cancer. Results The positive rates of COX I and COXm expression were 75. 0% (15/20) and 70. 0% (14/20) in normal colorectal tissue, 35.0% (7/20) and 25.0% (5/20) in eolorectal severe dyaplasia tissues and, 36. 2% (29/80) and 26. 2% (21/80) in colorectal cancer tissues, respectively. The expres- sion rates of the COX I and COX Ⅲ were significantly different between the colorectal normal tissue and colorectal severe dyaplasia tis- sues ( χ2 = 6. 465, P = 0. 011, χ2 = 8. 120, P = 0. 004 ), and between the colorectal normal tissue and colorecta] cancer (χ2 = 9.75, P = 0. 002,χ2 = 13.46, P 〈 0. 001 ). The expression level of the COX I in colorectal cancer tissues was associated with patient stage, and grade (P =0. 041, P =0. 017). The expression level of the COX Ⅲ was associated with patient stage (P =0. 008). Moreover, Kaplan-Meier survival analyses revealed that patients with the expression of the COX I and COX Ⅲ had significantly better survival ( P = 0. 008 and P = 0. 017, respectively). Conclusion COX I and COX Ⅲ are reduced in colorectal cancer, suggesting that the re- duced expression is associated with stage and grade. Furthermore, the expression of COX I and COX Ⅲ is correlated with colorectal cancer prognosis. COX I and COX Ⅲ could be used as potential prognostic markers.
