外阴硬化性萎缩性苔藓继发鳞状细胞癌1例

摘　　要：患者女,76岁。外阴增生物伴疼痛3个月。既往有硬化性萎缩性苔藓病史10余年。皮损组织病理示:角化过度,角化不全,表皮增生,表皮基底部细胞向下增生,细胞排列轻度紊乱,局部可见真皮浅层鳞状细胞团块,可见角化不良细胞及核异型性,真皮乳头层不等量淋巴细胞为主浸润。诊断:外阴鳞状细胞癌;外阴硬化性萎缩性苔藓。A 76-years-old female presented with painful vulvar neoplasia for 3 months. She had had lichen sclerosus et atrophicus for more than 10 years. The histopathological examination showed hyperkeratosis, dyskeratosis, epidermal hyperproliferation, downward extension of basal cell proliferation with irregular arrangement, partially visible squamous cell mass in the superficial dermis, dyskeratotic cells with atypical nuclei, and lymphocytic infiltration in the papillary layer of the dermis. Diagnose ： vulvar squamous cell carcinoma; lichen sclerosus et atrophicus.
