腹腔镜手术与开腹手术治疗进展期胃癌的对比分析

摘　　要：目的对比分析腹腔镜手术与开腹手术治疗进展期胃癌的效果,探讨腹腔镜手术对于进展期胃癌的临床价值。方法选取2011年6月至2013年7月安徽省肿瘤医院治疗的98例进展期胃癌患者作为研究对象,其中施行腹腔镜手术治疗的48例患者作为腹腔镜组,施行开腹手术治疗的50例患者作为开腹组,对两组患者的术中出血量、手术时间、清扫淋巴结数目、术后排气时间、住院时间、术后并发症及住院费用等指标进行比较。结果两组患者在手术时间、清扫淋巴结数目及住院费用方面比较差异无统计学意义(P>0.05),但在术中出血量、术后排气时间、术后并发症发生率及住院时间等方面比较,腹腔镜组显著低于开腹组(P<0.05)。结论腹腔镜治疗进展期胃癌具有出血量少、创伤小、恢复快等优点,其较开腹手术治疗近期优势明显,值得在临床推广应用。Objective To retrospectively analyze the effect of laparoscopie surgery and laparotomy in the treatment for advanced gastric carcinoma,and explore the clinical value of laparoscopic surgery in the treatment for advanced gastric carcinoma. Methods A total of 98 eases of patients with advanced gastric carcinoma admitted in Tumor Hospital of Anhui Province from Jun. 2011 to Jul. 2013 were selected as research objects,among which 48 patients treated with laparoscopie surgery were considered as members of laparoscope group and 50 patients treated with laparotomy were considered as members of laparotomy group. The intraoperative blood loss volume, operation time, number of dissected lymph nodes, postoperative exhaust time, hospitalization duration, postoperative complications and hospitalization expenses of the two groups were compared. Results There were no significant differences of the number of dissected lymph nodes, operation time and hospitalization expenses between the two groups （P 〉 0.05 ）, but the intraoperative blood loss volume, postoperative exhaust time, postoperative complications and hospitalization duration of the laparoscope group were significantly lower than the laparotomy group （ P 〈 0.05 ）. Conclusion Laparoseopic surgery treatment for advanced gastric carcinoma has the advantages of less bleeding, less trauma, quick recovery, its short-term effects are better than laparotomy,which is worth promoting in clinical practice.
