摘　　要：目的探讨人类表皮生长因子2(Her-2)、第10号染色体缺失的磷酸酶和张力蛋白同源物基因(PTEN)在胃腺癌组织中的表达及意义。方法应用免疫组织化学法检测临朐县人民医院2009年6月至2012年7月经手术常规病理证实的66例胃腺癌组织和25例正常胃组织中Her-2和PTEN的表达情况。结果 Her-2在癌组织中的阳性表达率明显高于正常组织,Her-2在浸润全层组的阳性率明显高于浸润黏膜层肌层组(45.5%vs 15.2%);Her-2在有淋巴结转移组的阳性率显著高于无淋巴结转移组(34.8%vs 25.8%)。PTEN在癌组织中的阳性表达率明显低于正常组织,PTEN在浸润全层组的阳性率显著低于浸润黏膜层肌层组(13.6%vs 22.7%);在有淋巴结转移组的阳性率显著低于无淋巴结转移组(7.6%vs 28.8%)。Her-2和PTEN在胃腺癌中的表达水平与患者年龄、性别、肿瘤直径和肿瘤的分化程度差异显著(P<0.05)。结论 Her-2的过表达与PTEN低表达或失表达,对胃癌的发生、发展及预后可能起重要作用。Objective To investigate the expression of Her-2 and PTEN in gastric cancer and its significance. Methods The expression of Her-2 and PTEN in 66 cases of patients with gastric cancer confirmed by general surgical pathology and in another 25 cases of normal gastric tissues admitted in Linqu People＇s Hospital from Jun. 2009 to Jul. 2012 were examined by immunohistochemistry（SP-method）. Results The positive rate of the expression of Her-2 in gastric cancer is higher than in the normal tissue. The positive rate of the expression of Her-2 in those cases the cancer invaded into serous membrane is higher than in those it invaded into mucosa（45.5% vs 15.2% ） ,and so there was obvious difference between the expression of Her-2 and the invasion depth. The positive rate of the expression of Her-2 in those cases with lympth node metastasis is higher than in those with no lympth node metastasis（34.8% vs 25.8% ）, so there was obvious difference between the expression of Her-2 and the lymph node metastasis. The positive rate of the expression of PTEN in those cases the cancer invaded into serous membrane is lower than in those it invaded into mucosa（ 13.6% vs 22.7% ）. The positive rate of the expression of PTEN in those cases with lympth node metastasis was lower than in those with no lympth node metastasis （7.6% vs 28.8% ）. Meanwhile, there were not differences between the expression levels of Her-2 and PTEN in patients with gastric cancer of different age and sex, tumor size and differentiation of tumor （ P 〈 0.05 ）. Conclusion Over-expression of Her-2 and low-expression or no expression of PTEN may play an important role in the incidence, progression and prognosis of gastric cancer.
