推拿结合特定电磁波谱照射治疗落枕的临床研究

摘　　要：目的:观察推拿结合特定电磁波谱照射治疗落枕的临床疗效,并探讨其对斜方肌状态的影响。方法:将符合要求的60例落枕患者随机分为2组,每组30例。治疗组采用推拿结合特定电磁波谱照射,对照组采用特定电磁波谱照射。治疗后参照《中医病证诊断疗效标准》中落枕的诊断标准评价疗效,并采用Myotonometer快速肌张力检测仪测定静息状态下斜方肌的肌肉张力。结果:1疗效评价结果。治疗组治愈15例、显效8例、有效7例,对照组治愈13例、显效8例、有效4例、无效5例,治疗组疗效优于对照组(Z=-2.561,P=0.010)。2斜方肌状态测定结果。2组患者治疗前的压力位移曲线下面积比较,差异无统计学意义[(10.69±2.08)mg·mL-1·min-1,(10.63±2.14)mg·mL-1·min-1,t=0.110,P=0.913];治疗后治疗组的压力位移曲线下面积较对照组显著增加[(16.90±1.61)mg·mL-1·min-1,(11.35±1.65)mg·mL-1·min-1,t=13.186,P=0.000]。结论:推拿结合特定电磁波谱照射治疗落枕,可以有效改善斜方肌状态,缓解肌紧张,有助于促进颈部功能恢复,疗效优于单纯的特定电磁波谱照射疗法。Objective：To observe the clinical curative effect of the manipulation therapy combined with specific electromagnetic spec- trum irradiation in the treatment of stiff neck and to explore its impact on the state of trapezius muscle. Methods： Sixty patients with stiff neck enrolled in the study were randomly divided into two groups ,30 cases in each group. Patients in treatment group were treated with manipulation therapy combined with specific electromagnetic spectrum irradiation, while the others in control group were treated with specific electromagnetic spectrum irradiation. The curative effect were evaluated after the treatment according to the diagnostic criteria of stiff neck which was extracted from Standard for diagnosis and therapeutic effectiveness evaluation of traditional Chinese medicine syndromes, and the trapezius muscle tonus were measured in quiescent condition by using Myotonometer. Results：Fifteen patients were cured ,8 got a good result and 7 fair in the treatment group;while 13 patients were cured,8 got a good result ,4 fair and 5 poor in the control group. The treatment group surpassed the control group in the curative effect（ Z = -2. 561 ,P = 0. 010）. There was no statistical difference in the area under pressure displacement curve （AUC）between the 2 groups before the treatment（ 10.69 ＋/-2.08 vs 10.63 ＋/-2.14 mg/ml/min,t = 0.110, P = 0. 913 ）. The AUC increased more significantly in the treatment group after treatment compared to the control group（ 16.90 ＋/- 1.61 vs 11.35 ＋/- 1.65 mg/ml/min, t = 13.186, P = 0. 000 ）. Conclusion：In the treatment of stiff neck, the therapy of manipulation combined with specific electromagnetic spectrum irradiation can effectively improve the trapezius muscle state, relieve muscular tension, and it is helpful to the neck functional recovery. Moreover,its therapeutic effect is better than that of monotherapy of specific electromagnetic spectrum irradiation.
