中国食管癌患者PTEN表达与临床病理因素相关性Meta分析

摘　　要：目的探讨中国食管癌患者第10号染色体磷酸酶和张力蛋白同源缺失基因（phosphatase and tensin homolog deleted on chromosome ten,PTEN）蛋白表达与肿瘤分化程度、浸润深度及淋巴结转移的相关性。方法在中国知网（CNKI）、中国生物医学文献数据库（CBM）及PubMed中,以＂食管癌、PTEN、分化程度、浸润深度和淋巴结转移＂等为检索词,检索2000-2013年已发表的文献资料,采用RevMan 5.0软件对纳入的文献资料进行加权定量合并,通过森林图和漏斗图分析食管癌患者PTEN蛋白表达与肿瘤分化程度、浸润深度及淋巴结转移的关系。结果 PTEN蛋白表达与分化程度Meta分析共纳入19篇文献,包括食管癌1 249例;PTEN蛋白阳性表达率在中高分化组是低分化组的2.39倍,RR=2.39,95%CI为2.01～2.85,z=9.73,P〈0.05。PTEN蛋白表达与肿瘤浸润深度Meta分析共纳入16篇文献,包括食管癌1 087例;PTEN蛋白阳性表达率在浸润侵及黏膜和黏膜下层是侵及深肌层和浆膜层组的1.83倍,RR=1.83,95%CI为1.62～2.07,z=9.87,P〈0.05。PTEN蛋白表达与淋巴结转移Meta分析共纳入19篇文献,包括食管癌1 293例;PTEN蛋白阳性表达率在无淋巴结转移组是有淋巴结转移组的1.92倍,RR=1.92,95%CI为1.55～2.38,z=6.02,P〈0.05。结论 PTEN蛋白的表达与肿瘤分化程度、浸润深度及淋巴结转移密切相关,PTEN可以作为临床评估食管癌恶性程度、浸润及淋巴结转移的重要指标。OBJECTIVE To explore relationship between expression of phosphatase and tensin homolog deleted on chromosome ten（PTEN）and degree of differentiation,depth of invasion and lymph node metastasis in Chinese esophageal cancer.METHODS Literatures published 2000-2013 were searched using ＂esophageal carcinoma,PTEN,differentiation,invasion,lymph node metastasis＂as the keywords.Statistical analysis was carried out by RevMan 5.0software,relationship between PTEN and degree of differentiation,depth of invasion and lymph node metastasis were analyzed through forest figure and funnel plot.RESULTS Total of 19 literatures were collected.About PTEN and tumor differentiation,including esophageal carcinoma patients 1 249,the results indicated positive rate of PTEN expression in medium and high differentiation group was 2.39 times than that of low differentiation group（RR=2.39,95%CI：2.01-2.85,z=9.73,P〈0.05）;Total of16 literatures were collected about PTEN and invasion depth,including esophageal carcinoma patients,results indicated positive rate of PTEN expression in mucosa and submucosa group is 1.83 times higher than in muscular and serosal layer group（RR=1.83,95%CI：1.62-2.07,z=9.87,P〈0.05）;Total of 19 literatures were collected about PTEN and lymph node metastasis,including esophageal carcinoma patients 1293,results indicated positive rate of PTEN expression in non-lymph node metastasis group is 1.92 times higher than lymph node metastasis group（RR=1.92,95%CI：1.55-2.38,z=6.02,P〈0.05）.CONCLUSION Relationship was observed about PTEN protein and malignant degree,invasion,metastasis and prognosis;PTEN can be used as an important index in evaluating the malignant degree,invasion and lymph node metastasis.
