摘  要:目的应用Meta分析方法研究E-cadherin蛋白在中国人前列腺癌组织中表达的临床意义。方法检索Pubmed、中国期刊网全文期刊数据库、维普和万方数据库,收集2013-10以前国内外专业期刊公开发表的文献、学位论文及学术会议的相关文献,根据前列腺癌标本的病理分级、临床分期以及E-cadherin蛋白检测和结果评价方法等,制定相应的纳入标准筛选文献。应用Meta分析软件RevMan 5.0对各研究原始数据进行统计处理,计算合并OR及95%CI,同时绘制Meta分析森林图和漏斗图。结果共有10篇文献纳入系统评价,对原始数据E-cadherin蛋白阳性表达率进行Meta分析,结果显示,正常前列腺组与前列腺癌组的OR为43.15,95%CI:5.79-321.30;前列腺增生组与前列腺癌组的OR为16.15,95%CI:8.78-29.69;前列腺癌高分化组与中、低分化组的OR为8.25,95%CI:4.45-15.32;前列腺癌A+B期与C+D期的OR为4.15,95%CI:2.64-6.52。前列腺癌组E-cadherin蛋白阳性表达率显著低于正常前列腺组和前列腺增生组,中、低分化组E-cadherin蛋白阳性表达率显著低于高分化组,C+D期E-cadherin蛋白阳性表达率显著低于A+B期。结论 E-cadherin蛋白表达降低可能在前列腺癌的发生和进展中发挥重要作用,E-cadherin蛋白表达与前列腺癌分化及临床分期密切相关。检测E-cadherin蛋白对指导前列腺癌临床诊治有重要参考意义。OBJECTIVE To evaluate the clinical significance of E-cadherin protein expression in prostate cancer of Chinese METHODS We conducted a systematic review of PubMed,CNKI,Weipu and Wanfang database before Oct.2013 to identify the related papers published in the professional journal,related dissertations and conferences papers.We established inclusion criteria according to the grade,clinical stage of prostate cancer and the method of detection and results evaluation for E-cadherin protein,and screened the papers accordingly.Meta analysis was conducted with RevMan5.0software.Merged odds ratio(OR)and 95% confidence interval(95%CI)were calculated.Also forest plots and funnel plots were worked out.RESULTS Ten papers were enrolled for the systematic evaluation.The Meta analysis was conducted to evaluate the positive rate of E-cadherin protein expression.The OR and 95%CI for different groups were listed below:normal prostate tissue and prostate cancer(OR:43.15;95%CI:5.79-321.30);benign prostatic hyperplasia and prostate cancer(OR:16.15;95%CI:8.78-29.69);highly differentiated and moderate,poor differentiated cancer(OR:8.25;95%CI:4.45-15.32);clinical stage A+ B and stage C+ D(OR:4.15;95%CI:2.64-6.52).The positive rate of E-cadherin in prostate cancer was significantly lower than that in normal prostate tissue and benign prostatic hyperplasia.The positive rate of E-cadherin in moderate and poor differentiated cancer was significantly lower than that in highly differentiated cancer.The positive rate of E-cadherin in clinical stage C+D was significantly lower than that in stage A+B.CONCLUSIONS The decrease of E-cadherin protein expression may play an important role in the carcinogenesis and progression of prostate cancer.E-cadherin protein expression is closely related to the differentiation and clinical stage of prostate cancer.The detection of E-cadherin protein is instructive for the diagnosis and therapy of prostate cancer.
