脱敏抛光膏治疗活髓牙牙体预备后牙本质敏感的疗效观察

摘　　要：目的探讨脱敏抛光膏对活髓基牙预备后牙本质敏感的临床疗效。方法 43例96颗行冠桥修复的活髓基牙在暂时冠桥及正式冠桥修复前分别用脱敏抛光膏处理,在疼痛游标尺上采用数字化疼痛评判法(VAS),记录处理前后的敏感程度(VAS值),采用SPSS 17.0软件包,对脱敏治疗前后VAS值采用配对t检验进行统计学分析。结果 43例患者(男19例,女24例)96颗活髓基牙,敏感位点分布在唇颊侧占43.37%;应用脱敏抛光膏后72.92%基牙症状即刻完全或部分缓解。暂时冠桥修复前未经脱敏抛光膏处理时的VAS值为(4.43±2.33),脱敏处理后VAS值为(2.23±1.85),处理前后差异有统计学意义(P<0.05)。正式冠桥修复前未经脱敏抛光膏处理的VAS值为(4.16±2.21),经脱敏处理后的VAS值为(1.99±1.83),处理前后差异有统计学意义(P<0.05)。96颗活髓基牙(男47颗,女49颗),暂时冠桥修复前的VAS平均值分别为3.48±2.65和4.16±2.62,差异无统计学意义(P>0.05)。结论活髓基牙预备后唇颊侧最敏感;脱敏抛光膏能够即刻缓解牙体预备后牙本质敏感,值得临床推广;活髓基牙预备后敏感程度与性别无关。【Objective】 To evaluate the clinical effects of desensitizing paste in providing instant relief of vital pulp abutment teeth dentin hypersensitivity. 【Methods】 Use Colgate desensitizing paste handling vital pulp abutment teeth before the cementation of provisional crowns and final dentures on 43 patients（96 abutment teeth）. Write down the hypersensitive measurements on VAS（Visualanalogue scale） and compare the values. 【Results】 A total of 43 subjects（19 male and 24 female） were enrolled in the study. Of 96 vital pulp abutment teeth studied, 40 were anterior teeth; 46 were premolars and molars. The main sensitive point on labial and buccal surface was 43.37%. There were significant reductions of 72.92% in probe stimulated hypersensitivity after desensitizing paste application. Before the cementation of provisional crown, using desensitizing paste made the VAS values drop from（4.43 ± 2.33 to 2.23 ± 1.85）（P 〈0.05）. Before the cementation of final denture, using desensitizing paste made the VAS values drop from（4.16 ± 2.21） to（1.99 ± 1.83）（P〈 0.05）. The VAS values of male（47 vital pulp abutment teeth） and female（49 vital pulp abutment teeth） were（3.48 ± 2.65） and（4.16 ± 2.62）, which was not statistical discrepancy（P〉0.05）. 【Conclusion】 Most of the sensitive points lie on labial or buccal surface. Desensitizing paste can provide an instant relief of vital pulp abutment teeth dentin hypersensitivity. Sex is not the influence of the vital pulp abutment teeth dentin hypersensitivity.
