腹腔镜辅助结直肠癌根治术治疗老年结直肠癌的效果分析

摘　　要：目的：分析腹腔镜辅助结直肠癌根治术治疗老年结直肠癌的临床效果。方法选择本院2012年3月～2014年3月收治的62例结直肠癌老年患者为研究对象，按照住院顺序随机分为对照组和观察组各31例，对照组患者行传统开腹直肠癌根治术，观察组患者采取腹腔镜辅助结直肠癌根治术，对两组手术情况、术中心肺功能及并发症情况进行比较。结果观察组手术时间、术中出血量、平均切口长度、术后排气时间及住院时间与对照组比较，差异有统计学意义（P＜0.05）；观察组患者术中射血分数（EF）、心脏指数（CI）、动脉血二氧化氮分压（PaCO2）、动脉血pH值及气道压力均明显高于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率为19.35%，对照组并发症发生率为45.16%，两组比较差异有统计学意义（P＜0.05）。结论相比传统开腹手术，腹腔镜辅助结直肠癌根治术具有术中出血量少、术中心肺功能稳定、术后并发症少、恢复快等特点，值得在老年结直肠癌患者中进一步应用。Objective To analyze the clinical effect of laparoscopy-assisted radical resection of colorectal cancer in the treatment of elderly patients with colorectal cancer. Methods 62 elderly patients with colorectal cancer admitted into our hospital from March 2012 to March 2014 were selected as research objects.They were evenly divided into observa-tion group and control group by the sequence of admission to the hospital.In control group,patients were treated by con-ventionally open radical resection for rectal cancer,while in the observation group,patients were treated by laparoscopy-assisted radical resection of colorectal cancer.The operation condition,intraoperative cardio-pulmonary function,and complication in the two groups were compared. Results In the observation group,operative time,blood loss,the average length of incision,postoperative discharge time and hospital stay compared with control group,the differences were sig-nificant （P＆lt;0.05）;Ejection fraction （EF）,cardiac index （CI）,arterial partial pressure of nitrogen dioxide （PaCO2）,arterial blood pH and airway pressure in the observation group were higher than those of control group,the differences were sig-nificant （P＆lt;0.05）.The incidence of complication in the observation group was 19.35%,while in the control group,it was 45.16%,with significant difference between the two groups （P＆lt;0.05）. Conclusion Compared with conventional laparoto-my,laparoscopy-assisted radical resection of colorectal cancer has advantages of less blood loss during surgery,more stable on intraoperative cardio-pulmonary function,few postoperative complications,and faster recovery,which is worthy of further application in elderly patients with colorectal cancer.
