胰十二指肠切除术治疗胰腺癌的效果评价

摘　　要：目的：探讨并评价胰十二指肠切除术（PD）治疗胰腺癌的临床效果。方法选取本院2008年1月~2010年1月收治的胰腺癌患者32例,将所有患者随机分为观察组和对照组,观察组行扩大PD,对照组行标准PD,并比较两组疗效。结果两组术中出血量、阳性淋巴结清扫个数、住院时间和并发症发生率、术后1年及3年生存率比较差异无统计学意义（P＆gt;0.05）,观察组手术时间长于对照组（P＜0.05）,总淋巴结清扫个数多于对照组（P＜0.05）,5年生存率高于对照组（P＜0.05）。结论与标准PD相比,扩大PD治疗胰腺癌可明显提高患者5年生存率,值得推广应用。Objective To explore and evaluate clinical efficacy of pancreaticoduodenectomy （PD）in the treatment of pancreatic cancer. Methods 32 patients with pancreatic cancer in our hospital from January 2008 to January 2010 were selected and randomly divided into observation group and control group,expand PD was used in observation group,stan-dard PD was applied in control group,and the effect of two groups was compared. Results The intraoperative blood loss, number of positive lymph node dissection,length of stay and incidence rate of complication and 1-year and 3-year sur-vival rate after surgery in two groups was compared respectively,with no statistical difference （P〉0.05）.The operative time in observation group was longer than that in control group （P〈0.05）,the total number of lymph node dissection in observation group was more than that in control group （P〈0.05）,5-year survival rate in observation group was higher than that in control group（P〈0.05）. Conclusion Compared with conventional PD,expand PD in the treatment of pancre-atic cancer can significantly improve 5-year survival rate of patients,is worthy of promotion and application.
