摘 要:目的探讨CXC型趋化因子配体16(CXC ligand 16,CXCL16)A181V多态性位点及其血清水平与脑梗死的病情严重程度和发病风险的关系。方法选择动脉粥样硬化性脑梗死患者120例为病例组,健康体检者75例为对照组。采用限制性片段长度多态性分析技术,检测2组CXCL16基因A181V多态性的分布。采用酶联免疫吸附法检测2组患者血清CXCL16水平。结果病例组CXCL16基因A181V的AA基因型(35.8%vs 20.0%)、A等位基因频率(59.6%vs 44.0%)明显高于对照组(P<0.05)。多因素logistic回归校正性别、年龄、吸烟、血糖、血压等因素后,CXCL16基因A181V的AA基因型脑梗死发病风险明显增加(P=0.015)。病例组血清CXCL16水平明显高于对照组[(0.25±0.20)ng/L vs(0.09±0.15)ng/L,P=0.000]。病例组和对照组CXCL16基因A181V的AA基因型患者血清CXCL16水平明显高于GG基因型患者(P<0.05)。CXCL16基因型与血清CXCL16呈正相关(r=0.359,P=0.000)。结论 CXCL16基因A181V的AA基因型增加动脉粥样硬化性脑梗死的发病风险,A等位基因可能是脑梗死的遗传易感基因之一,AA基因型能增加CXCL16表达。Objective To study the relationship of CXCL16 gene polymorphism and its serum level with atherosclerotic ischemic stroke and its risk factors.Methods One hundred and twenty atherosclerotic ischemic stroke patients served as a patient group and 75 individuals undergoing physical examination served as a control group in this study.The distribution of CXCL16 gene A181V polymorphism in 2 groups was analyzed by PCR-RFLP.The serum CXCL16 level was measured by ELISA.Results The frequency of AA genotype in CXCL16 gene A181V and A allele was significantly higher in patient group than in control group (35.8% vs 20.0%,59.6% vs 44.0%,P<0.05).The genotype of CXCL16 gene A181V was not related with the severity of ischemic stroke (P>0.05).After adjustment of age,smoking,blood glucose,blood pressure by multivariate logistic regression analysis,the incidence of ischemic stroke was higher in those with AA genetype of CXCL16 gene A181V (P=0.015).The serum CXCL16 level was significantly higher in patient group than in control group (P=0.000) and in patients with AA genotype of CXCL16 gene A181V than in those with GG genotype of CXCL16 gene A181V (P<0.05).The genotype of CXCL16 gene was positively related with the serum CXCL16 level (r=0.359,P=0.000).Conclusion The AA genetype of CXCL16 gene A181V increases the risk to develop atherosclerotic ischemic stroke.The AA allele is one of the genes susceptible to ischemic stroke and can upregulate the expression of CXCL16.
