进展期胃癌术中实施区域性缓释化疗安全性和远期疗效观察

摘　　要：目的对进展期胃癌术中患者实施区域性缓释化疗治疗,观察远期疗效和安全性。方法选择于2008年2月至2010年12月期间收治的104例进展期胃癌手术患者作为研究对象。按照患者手术时间先后顺序分组为对照组和观察组。对照组:实施常规治疗;观察组:实施区域性缓释化疗治疗。观察两组患者的远期疗效以及术后相关指标和不良反应。结果治疗后,观察组1、2、3年生存率分别为82.69%(43/52)、67.31%(35/52)、46.15%(24/52);对照组则分别为78.85%(41/52)、55.77%(29/52)、26.92%(14/52);两组1年生存率比较差异无显著性(P>0.05);而观察组2年生存率、3年生存率优于对照组,差异具有显著性(P<0.05)。观察组患者引流管流量高于对照组(89.5±7.1 ml/d vs.29.9±6.8 ml/d),引流管置管时间明显短于对照组(8.1±0.5 d vs.3.8±1.0 d),差异均具有显著性(P<0.05)。两组其他术后指标和不良反应比较,差异无显著性(P>0.05)。结论对进展期胃癌术中患者实施区域性缓释化疗治疗,其能够提高远期疗效,并且没有增加不良反应的发生率,是一种可靠且安全的治疗方法,具有一定的推广应用价值。Objective To investigate the long- term effect and safety of regional slow- release chemotherapy on advanced gastric carcinoma intraoperative period. Methods 104 patients with advanced gastric carcinoma collected from February 2008 to December 2010 were selected and divided into the control group and the observation group by sequence. The control group was given conventional therapy,while the observation group was given regional slow- release treatment. The long- term effect,indexes,and adverse reactions post- operation in both groups were observed. Results The one- year survival rate in the control group and the observation group was 82. 69%（ 43 /52） and 78. 85%（ 41 /52） respectively,without significant differences（ P〉0. 05）. The two- year and three- year survival rate in the control group and the observation group were 67. 31%（ 35 /52）,46. 15%（ 24 /52） and 55. 77%（ 29 /52）,26. 92%（ 14 /52） respectively,with significant differences（ P〈0. 05）. The flow and indwelling catheter time of drainage tube in the control group and the observation group were（ 89. 5 ± 7. 1） ml / d,（ 8. 1 ± 0. 5） d and（ 29. 9 ± 6. 8） ml/d,（ 3. 8 ± 1. 0） d,both with significant differences（ P〈0. 05）. No significant differences were detected among other indexes（ P〉0. 05）. Conclusion Regional slow- release chemotherapy given to advanced gastric carcinoma during operative period can increase the long- term effect,without increasing the incidence of adverse reactions. Therefore,it was a safe and reliable therapy,which is worthy of a wide clinical application.
