摘 要:目的研究膀胱癌患者尿沉渣中miR-142-3p和miR-145-5p差异性表达,为临床寻找早期膀胱癌肿瘤标志物提供依据。方法搜集51例未接受经尿道肿瘤切除术的早期膀胱癌患者和38例健康对照者的清晨中后段尿液,从尿沉渣中提取RNA,采用miRNA特异性茎环逆转录荧光定量PCR检测目标miRNA表达水平,分析两组的受试者工作曲线(ROC),比较miR-142-3p和miR-145-5p及其组合在膀胱癌早期诊断中的价值。结果 miRNA-142-3p在早期膀胱癌患者尿沉渣中显著升高(FC=3.93,P=0.0002,AUC=0.736)。组合测定miR-145-5p/miR-142-3p比值在膀胱癌患者与健康对照者间有统计学差异(P=0.00004,AUC=0.755)。结论测定尿沉渣中的miR-142-3p能有效发现早期膀胱癌,而检测miR-145-5p/miR-142-3p比值的诊断价值更高。Objective To investigate the contents of miR- 142- 3p and miR- 145- 5p in urine sediment of patients with bladder cancer. Methods Morning mid- stream urine samples were col ected from 51 patients with early stage bladder cancer before transurethral resection and 38 healthy controls. RNAs were extracted from urine sediment and targeting miRNAs were quantified by stem- loop qPCR. The area under receiver operating characteristic curve (AUC- ROC) was used to assess the di-agnostic value of miRNAs for bladder cancer. Results miR- 142- 3p in urine sediment was significantly elevated in urinary sediment of bladder cancer patients (FC=3.93, P=0.0002, AUC=0.736), while the miR- 145- 5p had a trend of reduction (FC=0.59, P=0.056). The ratio of miR- 145- 5p/miR- 142- 3p was significantly lower in early bladder cancer patients than that in healthy controls (FC=0.15, P=0.00004, AUC=0.755). Conclusion MiR- 142- 3p in urine sediment is increased in patients with early bladder cancer, and the ratio of miR- 145- 5p/miR- 142- 3p may be used for early detection of bladder cancer.
