摘　　要：目的探讨早期乳腺癌保乳手术中肿块切除的安全瘤缘距离。方法收集两家医院近年行肿块局部扩大切除术的84例早期乳腺癌保乳手术患者的临床资料及5年随诊资料,对切除肿块瘤缘距离为1cm的44例患者(A组)与2cm的40例患者(B组)的资料进行比较,对照分析两组患者的手术情况和预后。结果两组的手术时间、术中出血量及并发症发生率的差异均无统计学意义(均P>0.05),A组第1次切缘阳性患者的占比、局部复发及远处转移率均较B组为高,但两组的差异也均无统计学意义(均P>0.05)。结论为增加保乳手术的成功率,术中肿块切除的安全瘤缘距离应以2cm为宜。Objective To evaluate the safety distance of lumpectomy margin in breast- conserving surgery for early breast cancer Methods Clinical data of 84 patients with early breast cancer, who underwent breast- conserving surgery and were fol owed- up for 5 years, were retrospectively analyzed. There were 44 patients （group A） with the resection margin of 1 cm from the tumor edge and 40 patients （group B） with the resection margin of 2 cm. The correlation between surgical treatment and prognosis was analyzed in two groups. Results There were no significant differences in operation time, intraoperative blood loss and complication rate between two groups （P〉0.05）. The rates of positive margin, local recurrence and distant metastasis in group A seemed to be higher than those in group B, but there were not significant differences between groups （P〉0.05）. Conclusion To increase the success rate of breast- conserving surgery, the safe resection margin of 2cm from the tumor edge seems to be safer.
