摘 要:目的研究超声生物显微镜(ultrasoundbiomicroscopy,UBM)STS模式所测量高度近视患者睫状沟直径(ciliarysulcusdiameter,STS)与角膜水平直径(whitetowhitediameter,WTw)间的关系。方法选取2014年5—7月我院视光中心就诊的高度近视患者40例(74眼),根据不同前房深度,将患者分为浅前房(2.80mm≤ACD〈3.20mm)组,中等深度前房(3.20mm≤ACD〈3.40mm)组及深前房(ACD≥3.40mm)组,分别采用CompactTouchSTSUBM与IOLMaster测量患者睫状沟直径与角膜水平直径。结果三组不同前房深度患者所测得水平STS与肿差异有统计学意义(P值分别为0.000,0.013,0.000)。在浅前房组与中深度前房组,水平STS与w聊有显著相关性(Pearson相关分析,r值分别为0.439、0.578,P值分别为0.000、0.013),在深浅房组水平STS与wTw并未表现出显著相关性(r=-0.337,P=0.135)。三组中垂直睫状沟直径与水平睫状沟直径差异均有统计学意义(P值分别为0.000、0.008和0.000)。结论水平STS与wTw相关性较弱,且在不同前房深度两者的相关性不同,尤其在前房深度为3.40mm以上时,并不能以wTw预测STS的值。眼后房为竖椭圆形结构。全景UBM为ICL术人工晶状体尺寸选择及术前评估、术后拱高预测均可提供较为直观的依据。Objective To analyze the differences and correlation between ciliary sulcus diameters( STS) and horizontal cornea diameters (white to white, WTW). Methods Forty patients (74 eyes)with highly myopia visiting our hospital during May to July in 2014 were selected. According to anterior chamber depth ( ACD ), the patients were divided into shallow ACD group ( 2. 80mm to 3.20mm), medium ACD group( 3.21mm to 3.40mm)and deep ACD group( 〉3.40mm). The STS and WTW were measured by using acompact touch STS ultrasound biomicroscopy(UBM) and IOL Master, respectively. Results The horizontal STS and WTW in all the three groups exhibited significant differences ( P = 0. 0001,0. 013 and 0. 0001 ,respectively). In the shallow and medium ACD groups, the horizontal STS was significantly correlated with WTW( Pearson corre- lation analysis : r = 0. 439 and 0. 578, P = 0. 006 and 0. 024, respec- tively). In the deep ACD groups,there was no significant correlationbetween the horizontal STS and WTW( r =-0. 337 ,P = 0. 135 ). In addition,the verlical and horizontal STS showed significant differ-ences in till tile three groups. Conclusion At different ACDs,there are different correlations between tire horizontal STS and WTW. When ACD〉3.4mm, tile WTW is not a good predictor of horizontal STS. The posterior chambers were verlieal oval. The fllll sereen UBM provides mors inluitive basis for the ICL sizes decision,preoperative assessment and vaull predictive.
