摘  要:目的探讨还原型谷胱甘肽辅助治疗2型糖尿病合并非酒精性脂肪肝的临床疗效。方法将74例2型糖尿病合并非酒精性脂肪肝患者随机分为对照组及治疗组各37例,两组均予口服降糖药物和常规护肝药物治疗,治疗组加用还原型谷胱甘肽,静脉点滴治疗,治疗3周后,观察两组患者的谷草转氨酶(AST)、谷丙转氨酶(ALT)、谷酰转肽酶(GGT)、碱性磷酸酶(ALP)、甘油三酯(TG)、总胆固醇(TCHO)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、胰岛素抵抗指数(HOMA-IR)、超敏C反应蛋白(hs-CRP)等指标的变化。结果疗程结束后,治疗组AST、ALT、GGT、TG、TCHO、LDL-C、HOMA-IR、hs-CRP均降低,与本组治疗前相比,差异有统计学意义(P<0.01),且治疗组TG、TCHO、LDL-C、HOMA-IR、hs-CRP较对照组相比,差异有统计学意义(P<0.01);对照组仅AST、ALT下降,与治疗前相比差异有统计学意义(P<0.01)。结论对2型糖尿病合并非酒精性脂肪肝患者,还原型谷胱甘肽辅助治疗能有效改善肝酶学的异常,同时可调节血脂、抑制慢性炎症反应、改善胰岛素抵抗。Objective To study the curative effect of reduced glutathione on type 2 diabetes mellitus complicated with nonalcoholic fatty liver disease. Methods A total of 74 cases of type 2 diabetes mellitus complicated with nonalcoholic fatty liver disease were randomly divided into treatment group and control group,with 37 cases in each group. The treatment group was given conventional treatment combined with reduced glutathione, and the control group was given conventional treatment.After treatment for 3 weeks, the therapeutic effect was compared between the two groups.Results After treatment for 3 weeks,the liver function index, lipid levels, HOMA-IR, high-sensitivity C-reactive protein(Hs-CRP) of the treatment group were all significantly decreased(P0.01), and the differences of TG, TCHO, LDL-C,HOMA-IR, hs-CRP between the two groups were statistically significant(P0.01). Conclusion Conventional treatment combined with reduced glutathione in treatment of type 2 diabetes mellitus complicated with alcoholic fatty liver has good clinical effects,can improve liver function and regulate blood fat metabolism, it also can inhibit the chronic inflammation and improve insulin resistance.
