摘 要:目的 探讨甲状腺影像报告与数据系统(TIRADS)在甲状腺良恶性结节诊断中的价值.方法 对121例患者的145个甲状腺结节进行研究,根据恶性特征的多少将结节分为TIRADS2~5级.单因素与多因素分析超声表现与恶性结节的相关性.结果 145个结节中,有54个恶性,91个良性.TIRADS诊断恶性结节的敏感度为87.0%、特异度为69.2%、阳性预测值为62.6%、阴性预测值为90.0%、诊断准确性为75.9%.显著的低回声、纵向生长方式、不清晰的边界与微钙化是恶性结节的危险因素.结论 TIRADS有助于评价甲状腺结节的恶性风险,利于患者的临床处置.Objective To evaluate the applicability of sonographic thyroid imaging reporting and data system (TIRADS) classification for differentiating benign from malignant thyroid nodules.Methods 145 thyroid nodules in 121 patients with pathologic diagnosis were included in this study.All nodules were assigned to TIRADS classification ranged from 2 to 5 according to the level of suspicion for malignancy.Univariate and multivariate analyses were performed to investigate the relationship between suspicious ultrasound features and thyroid cancer.Results Of 145 nodules,54 were malignant and 91 were benign.Sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of TIRADS for the diagnosis of malignancy were 87.0%,69.2%,62.6%,90.0% and 75.9% respectively.Marked hypoechogenicity,taller than wide shape,irregular margins,and microcalcifications were risk factors of malignancy.Conclusion TIRADS may assess the risk of individual nodules being cancers and facilitate patient management.
