摘  要:目的脊柱血管瘤是一种常见椎体良性肿瘤,目前缺乏统一的治疗标准。本研究旨在探讨CT引导下经皮椎体成形术治疗脊柱椎体血管瘤的价值。方法脊柱血管瘤共26例,男12例,女14例;平均年龄52.1岁;肿瘤来源为:胸椎23椎,腰椎6椎,胸椎合并腰椎3例;在CT引导下对病变椎体经皮穿刺进入椎体并注入骨水泥。术前和术后不同时间段分别采用视觉模拟评分法(visual analogue scale,VAS)评分、活动能力评分以及远期疗效评估,并对结果进行统计学分析。结果 26例患者手术成功率为100%,有效率为96.1%。术后患者疼痛症状得到明显改善,术后VAS评分显著降低,由术前的(7.74±1.19)分降至术后6个月的(0.20±0.17)分,且活动能力评分及生活质量明显提高。结论 CT引导下椎体成形术可有效控制脊柱血管瘤引起的疼痛,增加脊柱的稳定性,是一种微创、安全、有效的治疗方法。Objective To discuss the primary efficacy of vertebral hemangiomas treated with the CT-guided percutaneous vertebroplasty(PVP).Methods 26 cases(12 males and 14 females) with 29 vertebral lesions with an average age of 52.1(range,35 -77) were and injected with bone cement percutaneous into the vertebral under CT guidance between April 2010 and May 2013.Visual analogue scale(VAS) were used to assess pain of patients and Oswestry disability index(ODI) were used to assess patients' life quality preoperatively and 3 d,14 d,180 d postoperatively.Results All cases were performed successfully.The success rate in all patients was 100%(26 /26) without serious complications.Postoperative score of VAS was significantly lower than preoperative score(P 0.05).The significantly difference could be observed among the preoperative and postoperative scores of ODI for each observed time point(P 0.05).Conclusion CT-guided percutaneous vertebroplasty(PVP) for vertebral hemangioma effectively relieve the pain caused by vertebral body and increases stability.PVP is a minimally invasive,effective,and safe treatment for symptomatic vertebral hemangioma.
