胶原酶消化法用于孕早期绒毛培养及产前诊断核型分析

摘　　要：目的比较胶原酶消化法与切碎贴壁法在孕早期产前诊断绒毛细胞培养中的细胞收获时间,分析孕早期绒毛染色体核型,比较异常核型发生率,探讨其在产前诊断中的应用价值。方法 2012年1月-2013年11月到本院就诊,符合产前诊断指征的早孕期孕妇121例,B超引导下经腹绒毛取材,同时或单独经胶原酶消化法与切碎贴壁法进行细胞培养,传代之后常规进行G显带,核型分析。计录收获时间,分析异常核型出现的种类及比率,比较不同年龄组的异常率。结果 134例胎儿绒毛,培养成功133例,成功率为99.25%。胶原酶消化法的收获时间明显快于切碎贴壁法,差异具有统计学意义(P<0.05)。结论采用胶原酶消化法,对产前诊断绒毛培养及核型分析具实用性。Objective To compare the different cells-harvesting time in isolation and culture of chorionic villi between collagenase digestion method with minced method,analyze karyotypes in early pregnancy,and compare the incidence rates of abnormal karyotype in the two methods so as to research its application value in prenatal diagnosis.Methods Samples from 121 pregnancy cases with 134 fetuses,according with the prenatal diagnosis indicatio,were taken under the guide of B ultrasound in the Third Affiliated Hospital of Guangzhou Medical University from January 2012 to December 2013.Collagenase digestion method and minced method were used respectively or simultaneously in the isolation and culture of chorionic villi samples.Then the chorionic villi cells were subcultrued.Their karyotypes were analysed by G band.Recorded the havesting time,analyzed the kinds,incidence ratio of abnormal karyotype,and compared the abnormal ratio in different ages.Results There were 133 successful cultured chorionic villus over all 134 cases,the success rate is 99.25%.Collagenase digestion methods have shorter harvest time than minced method.The difference was statistically significant（P〈0.05）.Conclusion Collagenase digestion is feasible in cultivating chorionic villus and analyzing karyotype in the prenatal diagnose.
