兔透明角膜切口术后高眼压对前房炎症反应的影响

摘　　要：目的探讨3.2mm无缝线透明角膜活瓣切口术后眼压与前房炎症反应的关系。方法 60只白兔其右眼为实验眼,随机分为高眼压组27眼（术毕眼压20~25mm Hg）、低眼压组27眼（术毕眼压10~15mm Hg）和对照组6眼。对照组不做任何处理,高眼压组和低眼压组兔眼作3.2mm无缝线透明角膜活瓣切口,分别于术后12、24h测定前房水白细胞计数,并进行眼内炎症的前房临床分级,并与对照组进行比较。结果术后12、24h,低眼压组前房水白细胞计数（2.000（0.000,5.625））×106/L、（1.000（0.000,2.500））×106/L）高于高眼压组（（0.000（0.000,0.000））×106/L、（0.000（0.000,0.000））×106/L）和对照组（（0.000（0.000,0.125））×106/L、（0.000（0.000,0.125））×106/L）（P均〈0.05）,高眼压组与对照组比较差异无统计学意义（P〉0.05）;术后12、24h,低眼压组1~2级房水闪辉眼数（12眼、10眼）均多于高眼压组（4眼、3眼）和对照组（0、0）（P〈0.05）,但高眼压组与对照组比较差异无统计学意义（P〈0.05）。结论 3.2mm无缝线透明角膜切口术后短期内适度高眼压可减少前房炎症反应。Objective To study the relationship between postoperative intraocular pressure（IOP）and inflammatory response in the anterior chamber after a 3.2-mm sutureless clear corneal incision.Methods Sixty right eyes of 60 rabbits were randomly divided into high IOP group（n=27）with IOP of 20 to 25mm Hg at the end of surgery,low IOP group（n=27）with IOP of 10 to 15mm Hg at the end of surgery,and control group（n=6）with no incision.Each eye received a 3.2-mm sutureless clear corneal incision.Aqueous humor concentrations of leukocytes were measured in postoperative12 and 24hours respectively,and the flare and inflammatory response of the anterior chamber was graded and compared with control group.Results The postoperative 12-and 24-hour leukocytes concentrations in aqueous humor were higher in low IOP group（2.000（0.000,5.625）×106/L）,（1.000（0.000,2.500）×106/L）than those in high IOP group（0.000（0.000,0.000）×106/L）,（0.000（0.000±0.000）×106/L）and control group（0.000（0.000,0.125）×106/L）,（0.000（0.000,0.125）×106/L）（P〈0.05）,and there were no significant differences between high IOP group and control group（P〉0.05）.The numbers of eyes with aqueous flare at grade 1to 2in postoperative 12 and 24hours were higher in low IOP group（12,10）than those in high IOP group（4,3）and control group（0,0）,and there was were no significant differences between high IOP group and control group（P〉0.05）.Conclusion The transient elevation of IOP after 3.2-mm clear sutureless corneal incision can reduce the inflammatory response in the rabbit’s anterior chamber.
