摘　　要：目的:检测细胞角蛋白CK18在胃癌组织及周围淋巴结中的表达,进一步探讨CK18对于胃癌诊断及预后评估的临床意义。方法:选取150例胃癌样本,包括黏液腺癌30例,乳头状腺癌34例,管状腺癌27例,低分化腺癌44例,印戒细胞癌15例。应用免疫组化技术检测胃癌组织及周围淋巴结中CK18的表达水平,阐明CK18的阳性表达与胃癌患者临床病理特征之间的相关性。结果:CK18的表达与患者的性别、年龄不存在相关性(P>0.05);胃癌组织的类型与CK18的表达无关(P>0.05);胃癌伴有淋巴结转移的患者中,CK18阳性率明显升高,差异有统计学意义(P<0.05);在胃癌伴有淋巴结转移的患者中,CK18的表达率与淋巴结转移的个数相关,差异有统计学意义(P<0.05)。结论:用免疫组化技术检测胃癌及周围淋巴结组织中CK18的表达情况,有助于胃癌预后的评估,提高胃癌淋巴结转移的检出率,为进一步判断TNM分期,完善胃癌术后的辅助治疗提供有力的依据。Objective：To detect the expression of cytokeratin 18 （ CK18 ） in gastric cancer and re-gional lymph nodes and explore the significance of CK 18 for the diagnosis and prognosis of gastric cancer .Methods：150 cases of gastric cancer were selected , including 30 cases of mucinous ade-nocarcinoma , 34 cases of papillary adenocarcinoma , 27 cases of tubular adenocarcinoma , 44 cases of poorly differentiated adenocarcinoma , 15 cases of signet ring cell carcinoma .Immunohistochem-istry was used to detect expression levels of CK 18 in gastric cancer tissue and regional lymph nodes to clarify the correlation between CK18 and clinicopathological features of gastric cancer .Results：The expression of CK18 was not correlated with gender , age of gastric cancer patients with or with-out metastatic lymph nodes （ P〉0 .05 ） .The types of tumor was not correlated with CK 18 （ P〉0 .05 ） .The positive rate of CK 18 was significantly increased in gastric cancer with lymph node metastasis, the difference was statistically significant （P〈0.05）.In gastric cancer patients with metastatic lymph nodes , the expression of CK18 was correlated with the number of metastatic lymph nodes.The difference was statistically significant （P〈0.05）.Conclusion：The expression of CK18 in gastric cancer and regional lymph nodes detected by immunohistochemical method could be helpful to assess the prognosis of gastric cancer and improve the detection rate of metastat -ic lymph nodes .It could provide the basis for establishing TNM stage and improving the adjuvant treatment after operation .
