摘 要:目的:比较三种建立兔VX2肝癌模型方法的效果差异,评价超声在监测兔VX2肝癌中的价值,探讨建立兔VX2肝癌模型的最佳方法。方法:18只新西兰大白兔,随机分为三组:A组超声引导注入肿瘤组织块悬液;B组超声引导经皮穿刺种植肿瘤组织块;C组开腹手术瘤块种植法。分别于接种后第7、14、21和28天采用超声观测三组兔肝脏成瘤情况并记录,处死动物后取病理标本进行对照,比较不同方式肿瘤的生长特点。结果:超声观测下兔VX2肝癌多呈低或等回声结节,边界较清楚,血供较丰富,部分可见声晕。当肿块直径>1cm时,超声能准确地反映肿瘤生长情况,与病理检测结果一致。A组多为多结节散在分布,异位种植多;B组肿瘤多生长局限,多突出于肝表面被膜;C组肿瘤呈孤立结节,位于肝实质内。A、B、C三组接种成瘤率分别为:61.1%、44.4%、100%,差异有统计学意义(P<0.05)。单发率:11.1%、61.1%、88.9%。异位种植率:55.6%、38.9%、0%。平均存活时间:A组(38.2±4.5)d、B组(37.5±3.6)d、C组(41.2±3.5)d,差异无统计学意义(P>0.05)。结论:三种方法均能制成肝癌模型,开腹手术瘤块种植法成功率高、模型性质稳定,是可推荐的建立兔肝癌模型的方法,超声检查是一种监测肝癌模型的有效手段。Objective: To discuss three methods of making rabbit model bearing VX2 liver tumor and evaluate the application by ultrasound. Methods: All 18 New Zealand white rabbits were divided into three groups randomly,6 in each group. VX2 tumor pieces were punctured from skin into the liver by ultrasonic guidance in group A and VX2 tumor pieces mixed with sodium chloride were injected into the liver by ultrasonic guidance in group B,and VX2 tumor pieces were implanted into the liver after abdominal operation in group C. The growth features of the tumor established in three different ways were compared by ultrasound at 7,14,21,28 days after implantation or injection of VX2 tumor. Results: Most rabbit VX2 liver tumors presented as low- echoic nodules with clear border,some having surrounding echo halo or rich blood supply under sonography. When the masses diameter 1cm,ultrasound could reflect the growth of tumor accurately with the pathologic findings. Success rates of A,B,C groups were 61. 1%,44. 4%and 100% respectively,with significant statistical difference( P 0. 05). Single nodule rates of A,B,C groups were11. 1%,61. 1% and 88. 9%. Most masses appeared as disseminating multiple nodules with ectopic implantation in group A,and as localized growth,single nodule in group B and group C. Conclusion: All three methods can be used in making rabbit model bearing VX2 liver tumor,and the method of abdominal operation is the most effective. US is useful in evaluating liver tumor.
