摘  要:目的:研究孕烷受体(PXR)蛋白及多药耐药基因(MDR1)蛋白在小细胞肺癌化疗前后外周血单个核细胞(PBMCs)中的表达,探讨两者的相关性及其临床意义。方法:选取43例小细胞肺癌患者为实验组,20例健康人群为对照组,采用液基薄层离心涂片法分离PBMCs,SP免疫组织化学化方法检测PBMCs中PXR及MDR1的蛋白表达水平,并比较小细胞肺癌患者化疗前后PBMCs中PXR及MDR1的表达变化。结果:化疗前小细胞肺癌患者PXR、MDR1的蛋白表达与对照组相比,差异无统计学意义(P>0.05)。化疗后小细胞肺癌患者PXR和MDR1的蛋白表达较化疗前增加,且两者的表达差异均具有显著性意义。化疗前后小细胞肺癌患者PBMCs中PXR和MDR1的表达均呈正相关。结论:PBMCs中PXR、MDR1的表达与小细胞肺癌耐药相关,外周血PXR高表达可能是诱导化疗相关的MDR1表达升高的原因,外周血的检测可为早期判断小细胞肺癌临床耐药提供一种更为简单易行的方法。Objective: To investigate the expression of pregnane X receptor( PXR) and multidrug resistant protein- 1( MDR1) in peripheral blood mononuclear cells( PBMCs) of small cell lung cancer patients and their relationship before and after chemotherapy. Methods: All 43 cases of small cell lung cancer patients and 20 cases normal controls were involved in this study. After separated by centrifuge,combining with thinprep cytologic test,immunohistochemical methods were used to study the expression of PXR and MDR1 in PBMCs. Results: The expression of PXR and MDR1 in PBMCs of small cell lung cancer patients was not statistically significant compared with normal controls( P 0. 05). Significant increases of PXR and MDR1 in PBMCs were found after chemotherapy. The expression of PXR and MDR1 has significant positive correlation before and after chemotherapy. Conclusion: PXR and MDR1 expression in PBMCs may reflect the patient's drug resistance phenotype and provide an easier method for MDR judgement in early stage of small cell lung cancer.
