摘  要:目的探讨虚拟现实技术在垂体瘤术前规划中的作用。方法收集31例拟采用手术治疗的垂体瘤病人作为实验组,将术前采集的CTA及MRI数据输入Dextroscope虚拟现实系统进行三维重建,获得肿瘤、脑组织、骨质以及血管信息,并进行观察、测量、手术模拟等操作,制定手术规划。收集常规手术的20例垂体瘤病人作为对照组,比较两组手术时间、手术全切率以及KPS预后评分。结果实验组31例垂体瘤病人成功实现3D重建,与术中实际情况吻合。与对照组相比,实验组手术时间缩短,统计学有明显差异(P<0.05),手术全切率及KPS评分统计学无明显差异(P>0.1)。结论 Dextroscope虚拟现实系统可以提供肿瘤及其毗邻结构的信息,为个体化制定手术方案提供帮助,有助于提高手术的安全性。Objective To explore the role of virtual reality technique in preoperative planning of pituitary adenoma. Methods Thirty-one patients with pituitary adenoma were collected as experimental group, whose multiple imaging data including CTA and MRI were acquired before surgery and transferred to the Dextroscope system for 3-D reconstruction. The information of tumor, brain tissue, bones and vessels were obtained, observed and measured. Based on the above, simulated surgery was performed in order to make the surgical plan. The routine surgery was performed on 20 patients with pituitary adenoma as control group. The surgery time, rate of total removal and KPS scores between 2 groups were compared. Results 3-D reconstruction was successfully performed on 31 patients in the experimental group and the results were in accordance with surgical findings. Compared with control group, surgery time was shorter in experimental group (P〈 0.05). There were no significant differences in rate of total removal and KPS scores between the two groups (P 〉0.1). Conclusions Virtual reality technique can offer comprehensive information about the tumor and adjacent structures, offer guidance for making individualized surgical plan and help to increase the safety of surgery.
