肝细胞癌CT表现、肝外转移与病理分级和P53蛋白表达的关系

摘　　要：目的探讨肝细胞癌(HCC)CT表现、肝外转移与病理分级和P53蛋白表达的关系,进一步认识其CT征象的意义。方法经手术病理证实的HCC 60例,将其标本切片进行HE和P53基因蛋白染色,统计分析HCC的CT表现与肿瘤病理分级和P53表达的关系。结果 60例HCC,肿瘤包膜与病理分级有关(P<0.05),而肿瘤大小、瘤内坏死、门静脉瘤栓和肝外转移与病理分级无关(P>0.05)。而对于P53,肿瘤大小、包膜有无、瘤内坏死、门静脉瘤栓和肝外转移均与之无关(P>0.05)。结论 HCC所表现的包膜与肿瘤病理分级有关,是术前设计治疗方案重要依据之一。Objective To explore the relationship between P53 protein expression and CT findings,extrahepatic metastasis as well as pathological grade in hepatocellular carcinoma（ HCC） in order to make a further understanding of the significance of the CT features. Methods A total of 60 patients with pathologically-proved HCC were enrolled in this study.The pathological slices were prepared. Immunohistochemical staining was used to estimate the positive expression rate of P53 protein and HE staining was performed to determine the pathologic grade. The correlation of P53 protein expression with CT findings and pathological grade was statistically analyzed by using SPSS 13. 0. Results In the 60 HCC patients,the presence of tumor capsule was significantly related to the pathological grade（ P 〈0. 05）,while the tumor＇s size and the presence of necrosis,portal cancerous thrombus and extrahepatic metastasis had no definite relationship with the pathological grade（ P 〉0. 05）. No obvious correlation existed between P53 protein expression and CT features,including the tumor＇s size and the presence or absence of tumor capsule,necrosis,portal cancerous thrombus and extrahepatic metastasis（ P〈 0. 05）. Conclusion The tumor capsule of HCC is well related to its pathological grade,which is very helpful in designing the therapeutic scheme before operation.
