美金刚联合丁苯酞治疗血管性痴呆临床疗效观察

摘　　要：目的:观察美金刚(易倍申)和丁苯酞(恩必普)对血管性痴呆(Va D)患者认知和行为功能障碍的改善作用及安全性。方法:将40例Va D患者随机分为美金刚联合丁苯酞治疗组22例(联合治疗组)和美金刚治疗组18例(对照组)。2组患者分别在治疗前和治疗3个月、6个月进行简易精神状态检查(MMSE)以及日常生活能力(ADL)的测评,比较2组的评分结果,并观察不良反应。结果:联合治疗组患者3个月和6个月ADL评分分别为(25.2±5.4)分和(23.2±5.6)分,MMSE评分分别为(18.8±4.4)分和(20.1±4.8)分;对照组患者3个月和6个月ADL评分分别为(26.6±5.9)分和(25.4±6.1)分,MMSE评分分别为(18.1±4.9)分和(18.9±4.7)分。2组与治疗前相比差异均有统计学意义(均P<0.05)。2组间治疗后6个月比较差异有统计学意义(P<0.05)。2组均未出现严重不良反应。结论:美金刚和丁苯酞联合应用对Va D的认知功能、行为能力改善效果较单用美金刚明显,并具有较高的安全性。Objective： To observe the curative effect and safety of combination of memantine and bu-tylphthalide in the treatment of vascular dementia （VaD）, the improvement of the cognitive function and behav- ioral capacity. Methods： Forty cases with VaD were randomly divided into 2 groups： one group treated with memantine and butylphthalide, the other group treated with memantine. MMSE scale and ADL scale of 2 groups were assessed and compared the pre-treatment with the treatment in 3 months, 6 months later. A comparative study was made to evalate the curative effects and side effects between 2 groups. Results： After treating with me- mantine and butylphthaliden for 3 months and 6 months, the ADL scores were （25.2±5.4） and （23.2±5.6） respec- tively, MMSE scores were （18.8±4.4） and （20.1±4.8） respectively. After treatment with memantine for 3 months and 6 months, the ADL scores were （26.6±5.9） and （25.4±6.1） respectively, MMSE scores were （18.1±4.9） and （18.9±4.7） respectively. The data compared pre-treatment with the two groups had statistical significance （P〈0.05）. The scores of MMSE and ADL after 6 months treatment between two groups have statistical signifi- cance （P〈0.05）. No serious adverse events was recorded during treatment. Conclusion： Combination of meman- tine and butylphthalide can markedly ameliorate the cognitive function and behavioral abilities in patients with vascular dementia. Its combination with butylphthalide is superior to its single use with high safety.
