摘  要:目的 探讨虚拟现实(VR)技术在鞍区肿瘤术前规划制定中的作用.方法 87例拟采用手术治疗的鞍区肿瘤患者,分别采集多种医学影像数据,输入Dextroscop术前计划系统,在VR环境中还原为一个空间立体的三维物像.从而在术前充分获得肿瘤,脑组织,颅底骨质以及神经血管全面而直接的信息,再利用工具进行观察,测量、手术模拟等操作,制定周密的手术规划.并与未行虚拟规划的20鞍区肿瘤病例进行比较手术时间,手术全切率,并发症发生率以及随访后的KPS预后评分,视力恢复情况.结果 87例病例均成功地实现了三维立体虚似现实影像的重建,与术中实际情况吻合.与对照组相比,手术时间明显缩短,并发症发生率较少,视力恢复情况较好,有明显差异(P<0.05),KPS评分及手术全切率差异无统计学意义(P>0.1).结论 Dextroscope-统的虚拟现实技术能快速、直观、全面地整合鞍区肿瘤多种医学影像数据,以提供肿瘤及其毗邻解剖结构的综合信息,在手术前为制定和优化手术方案提供帮助,有可能有助于提高了手术的安全性.Objective to evaluate the usefulness of virtual reality technique in preoperative planning of sellar tumor.Methods Multiple medical imaging data collected from 87 patients who suffered sellar tumor,were transferred to Dextroscope system.A stereoscopic 3-D object displayed after reconstructing the data with the Radiodexter software in virtual reality environment.A suite of 3-D tools accessible inside the Dextroscope workspace enabled users to obtain measurement and simulated intraoperative viewpoint about the lesion and adjacent anatomic structure A sophisticated preoperative plan was defined.Operative time,total resection rate,complication rate,KPS score and visual recovery rate of 87 cases were compared with control group 20 cases suffered from sellar tumor without virtual preoperative plan.Results 3-D stereoscopic virtual reality images of 87 cases were reconstructed successfully,and of coincidence with real situation in operations.Virtual preoperative plan could shorten operative time,reduce the complication rate,increase the visual recovery rate (P <005),but could not improve the total resection rate and the KPS score(P >0.1),compared with control group.Conclusions VR technique in Dextroscope system can integrate multiple medical imaging data quickly,intuitively and overall,offer a comprehensive information about the sellar lesions and related structure anatomy,give a hand in optimizing the operative project and might potentially increase the safety of operation.
