摘　　要：目的 探讨5-HT转运蛋白基因启动子区域（5-HT-TLPR）多态性与腹泻型肠易激综合征（D-IBS）相关性.方法 计算机检索Pubmed、Embase、Web of science、CBM、CNKI和万方等数据库,查找5-HTTLPR多态性与D-IBS发病风险相关性的病例对照研究,检索时限均从建库至2013年9月.按照纳入标准筛选文献、提取资料后,采用Stata 12.0软件进行Meta分析.结果 最终纳入14个研究,包括1044例患者及2 147例对照者.Meta分析结果显示：在白种人群中,5-HTTLPR与D-IBS发病存在相关性,携带LL基因型人群发生D-IBS风险较高[LLvsLS：OR=1.46,95％ CI（1.15～ 1.86）;LL vs LS＋ SS：OR=1.26,95％ CI（1.01 ～ 1.57）],而在亚洲人群中,5-HTTLPR与D-IBS发病无相关性.结论 基于本研究结果,5-HTTLPR多态性与D-IBS的易感性有人种差异,在白种人中,LL基因型携带者患D-IBS风险增高,但上诉结论仍需开展更多高质量、大样本的临床试验以进一步验证.Objective To evaluate the correlation of 5-HTTLPR polymrphism and diarrhea-predominant irritable bowel syndrome.Methods Databases including Pubmed,Embase,Web of science,CBM,CNKI and WanFang Data were searched from inception to September 2013 to collect case-control study on the correlation of 5-HTTLPR polymorphism and D-IBS.According to the inclusion criteria,data of the included studies were extracted,and the metaanalysis was performed using Stata 12.0.Results A total of 14 studies were included,involving 1 044 D-IBS cases and 2 147 controls.The results of meta-analysis showed that there were significant differences in Caucasian,and the LL genotype carriers tended to face an increasing D-IBS risk[LL vs LS：OR =1.46,95％ CI（1.15 ～ 1.86） ;LL vs LS ＋ SS：OR =1.26,95％ CI（1.01 ～ 1.57）],but among Asian populations,there was no significant difference in the correlation between 5-HTTLPR polymorphism and D-IBS.Conclusion Based on the results,5-HTTLPR polymorphisms may play a role in the susceptibility of D-IBS in an ethnicity specific manner.The Caucasian who carriers LL genotype tends to face an increasing D-IBS risk,this conclusion has to be proved by more studies.
