年龄影响增生性糖尿病视网膜病变患者手术预后

摘　　要：目的 观察近1年内发病的增生型糖尿病视网膜病变（PDR）患者的年龄对玻璃体视网膜手术预后的影响.方法 回顾分析2009年5月至2012年10月期间120例（均以住院时间为止）,近1年内发生PDR需接受玻璃体视网膜手术治疗的2型DM患者的临床资料,其中30～50岁年龄段60只眼,51～70岁年龄段60只眼.手术后平均随访20个月.通过两组患者术中实际视网膜增殖情况,术后硅油填充率,术后玻璃体再出血率,术后FFA检查结果等观察指标进行分析比较.结果 手术前30～50岁年龄组Ⅴ期PDR患者术中实际存在局部牵拉性视网膜浅脱离者占16.67％;手术治疗后眼内填充物为硅油者占13.53％;手术治疗后再合并玻璃体积血者占11.54％;手术治疗后3个月至6个月FFA检查结果视网膜仍见至少两个象限存在新生血管团者占21.15％;均高于51～70岁年龄组,有统计学意义.结论 相同分期下,30～50岁PDR患者视网膜增殖程度,术后再增殖率均明显高于51～70岁PDR年龄组患者;PDR患者的发病年龄早晚对玻璃体视网膜手术的预后有一定影响.Objective Observation on the influence of the age of the patients of Proliferative Diabetic Retinopathy （PDR） who in the past 1 year had been through vitreoretinal surgery. Methods Retrospective analysis of 120 cases during the May 2009 to October 2012 （were based on length of stay）, of which the patients,whose PDR was subjected to vitreo-retinal surgery clinical data with type 2 DM, of 30-50 age 60 eyes, 51 - 70 age 60 eyes. All cases were followed up 20 months on average afcer swrgery. The proliferative conditions of retina during operation, the post operative silicone oil filling rate, the post-operative vitreous rebleeding rate and {luorescein fundus angiography changes were observed and compared between two groups above. Result the local traction retinal detachment rate was 16.67% in 30 to 50 age group with PDR in stage five; The post-operative silicone oil filling rate was 13. 53% ; the post operative vitreous rebleeding rate was 11.54%; the post-operative developing rate of neovessels in at least two quadrants of retina was 21.15% via fluorescein fundus angiography examination after a follow-up of 3 to 6 months. The differences between these groups was significant statistically （P 〈 0. 05）. Conclusion the post-operative retinal proliferation rate of 30 to 50 age group is higher than that of 51 to 70 age group under the same PDR stage. The diabetes developing age can impact the outcomes of proliferative diabetic retinopathy undergone vitrectomy to some extend.
