摘 要:目的探讨磁共振波谱成像(1 H-MRS)在肝硬化诊断与分期中的价值。方法选取30例不同程度的肝硬化患者(child A 15例,child B 8例,child C 7例)作为病例组和正常人30例作为对照组,对其行磁共振单体素1 HMRS研究,测算病例组和对照组患者的相关代谢物(Lip,Glx,Cho,Glyu)平均峰值,并进行统计学分析。结果 1与正常对照组相比,肝硬化患者的Glx和Cho峰值均高于对照组,Lip峰值低于对照组,且差异均有统计学意义(P<0.05);2不同程度肝硬化病例组内对比,Glyu、Cho和Glx的峰值均随着肝硬化程度升高而增大,Lip峰值随着肝硬化程度升高而减小,且Lip和Cho峰值的差异在child A、child B和child C组中均有统计学意义(P<0.05),而Glyu和Glx峰值差异均无统计学意义(P>0.05)。结论磁共振波谱成像(1 H-MRS)作为一种无创性的评价手段,其所测代谢物峰值在一定程度上可以对肝硬化的诊断与分期做出较为准确的评估,为临床早期肝硬化的诊断及评估提供可靠的影像学依据。Objective To evaluate the value of MR Spectroscopy(1 H-MRS)in diagnosis and stage of liver cirrhosis.Methods Thirty patients with liver fibrosis(child A15 cases,child B 8cases and child C 7cases)and thirty normal patients were included.They were examined by GE Propeller HD 1.5T MRI system in Prob-P.The metabolism value(Lip,Glx,Cho,Glyu)in liver are calculated and statistically analyzed.Results 1The value of choline and glutamine/glutamate in liver cirrhosis are higher but the value of lipid is lower.A significant statistical difference was observed between control vs.liver cirrhosis in choline and glutamine/glutamate and lipid(P〈0.05),whereas no significant statistical difference was observed in glycogen.2Among different child groups,the value of choline,glutamine/glutamate and glycogen increase as the degree of liver cirrhosis,but the value of lipid decrease as the degree of liver cirrhosis.A significantly statistical difference was observed between A vs.B,B vs.C and A vs.C in choline and lipid(P〈0.05),however,no significant statistical difference was observed in glycogen and glutamine/glutamate.Conclusion MRS can be used in diagnosis and stage of liver cirrhosis as a noninvasive means,and provide reliable imaging basis in clinical diagnosis of liver cirrhosis.
