摘 要:目的旨在研究细胞角蛋白(CK)及CD68对胃肠道淋巴结微转移的诊断价值。方法采用免疫组织化学PV-9000法检测41枚微小转移癌胃肠癌淋巴结(实验组)和28枚正常淋巴结(对照组)中的CK及CD68蛋白的表达。结果 CK在实验组的阳性率为85%(35/41),显著高于对照组14%(4/28)(P<0.01);CD68在实验组的阳性率为20%(8/41),显著低于对照组68%(19/28)(P<0.01)。Spearman分析提示在胃肠癌淋巴结微小转移组织中CK表达与CD68表达呈高度负相关(r=-0.841,P<0.01)。结论免疫组织化学法检测淋巴结微转移可以作为肿瘤细胞淋巴播散的指标。CK及CD68两者联合检测可应用于胃肠道淋巴结微小转移癌的检测。Objective To study the diagnostic value of CK and CD68 in gastrointestinal lymph node micrometas-tases. Methods Forty-one cases of gastriccancer lymph node micrometastases (experimental group) and 28 cases of normal lymph node (the control group) were detected by immunohistochemistry PV-9000 method for the expression of CK and CD68 protein. Results The positive rate of CK in the experimental group was 85%(35/41), which was signifi-cantly higher than the control group by 14%(4/28)(P〈0.01); the positive rate of CD68 in the experimental group was 20%(8/41), which was significantly lower than the control group by 68%(19/28)(P〈0.01). Spearman analysis showed that lymph node micrometastases in gastrointestinal cancer tissue CK expression and CD68 expression was negatively correlated (r=-0.841, P〈0.01). Con-clusion Immunohistochemistry was used to detect micrometastases in lymph nodes can be used as indicators of lymphatic spread of tumor cells. CK and CD68 can be used in combined detection of lymph node micrometastases gastrointestinal cancer detection.
