脑血管疾病检测血栓弹力图、纤维蛋白原及D-二聚体的相关性探讨

摘　　要：目的探讨脑血管疾病患者联合检测血栓弹力图(TEG)和纤维蛋白原(Fbg)及D二聚体(D-D)间的相关性。方法选取住院的脑梗死组病人47例和脑梗死合并心绞痛组病人35例,以及健康对照组50例,应用CFMS血栓弹力图检测仪和STAR全自动凝血分析仪分别检测TEG(K值、α角、MA值)和Fbg、D-D等项目。结果脑梗死组和脑梗死合并心绞痛组患者的Fbg与K值呈负相关(r=-0.128、P=0.000),Fbg与α角、MA值呈正相关(r=0.221、P=0.000;r=0.247、P=0.000);D-D与K值呈负相关(r=-0.134、P=0.000),D-D与α角、MA值呈正相关(r=0.246、P=0.000;r=0.252、P=0.000)。两两进行比较,发现脑梗死组、脑梗死合并心绞痛组与对照组的α角、MA值、Fbg、D-D水平均明显高于对照组(P<0.05);K值则明显低于对照组(P<0.05);脑梗死组与脑梗死合并心绞痛组之间K值、α角、MA值、Fbg、D-D水平无明显变化。结论脑梗死与脑梗死合并心绞痛的脑血管疾病的发生、发展与Fbg、D-D水平呈正相关,而Fbg、D-D与K值呈负相关;Fbg、D-D与α角、MA值呈正相关;所以TEG检测对于脑血管疾病来说是一种有实用价值的筛选试验,且TEG中的α角、MA值也是一个敏感反映脑血管疾病时凝血的指标,也可以为临床治疗提供有效的依据。Objective To discuss the correlation of fhrombelastograph,fibrinogen （Fbg） and D-dimer （D-D） in cerebrovascular disease.Methods We select 47 patients of cerebral infarction,35 patients of cerebral infarction merger angina in hospital and 50 of health people.We detect TEG （K value,α horn,MA value）,Fbg,D-D by using thrombelastograph and STAR automatic blood coagulation analyzer.Results Fbg is negatively correlated with K value and is positively correlated with α horn,MA value （P =0.000） in cerebral infarction and cerebral infarction merger angina.D-D is negatively correlated with K value and is positively correlated with α horn,MA value.Compared with the control group,α horn,MA,Fbg,D-D are significantly higher（P 〈 0.05）,and K value is significantly lower（P 〈 0.05）.There is no difference of K value,α horn,MA,Fbg,D-D between cerebral inffarction and cerebral infarction merger angina.Conclusion The occurrence and development of cerebrovascular diseases in cerebral infarction and cerebral infarction merger angina is positively correlated with the level of Fbg,D-D But,Fbg,D-D are negatively correlated with K value,and are positively correlated with α horn,MA value.So TEG is a useful screening test for detecting cerebrovascular disease.The α horn,MA value of TEG are also as a sensitive reaction of coagulation in cerebrovascular disease,which also can provide effective basis for clinical treatment.
