摘 要:目的探讨转移性乳腺癌患者血清糖类抗原15-3(CA15-3)和癌胚抗原(CEA)水平与临床病理参数的关系。方法收集2007年1月至2012年12月皖南医学院第一附属医院收治的转移性乳腺癌患者284例,所有患者在诊断为转移性乳腺癌时,均检测血清CA15-3和CEA水平,统计分析血清CA15-3和CEA水平与患者临床病理参数的关系。结果 284例患者中,血清CA15-3高于正常水平者163例(57.4%),血清CEA高于正常水平者97例(34.2%)。血清CA15-3和CEA水平升高与乳腺癌分子分型有关(均P<0.05)。Luminal亚型患者血清CA15-3和CEA升高比例显著高于HER2-enriched亚型及TN亚型。血清CA15-3升高与骨转移有关(P<0.05),而血清CEA升高与转移部位无关。多处转移的患者血清CA15-3升高比例高于单处转移的患者(P<0.05),而血清CEA升高比例在单处转移的患者和多处转移的患者无明显差异(P>0.05)。结论转移性乳腺癌患者中,CA15-3、CEA升高与乳腺癌分子亚型显著相关,骨转移的患者CA15-3升高比例高于其他转移部位,而CEA升高与转移部位无关。多处转移的患者CA15-3升高比例高于单处转移的患者。Objective To investigate the relationships between serum carbohydrate antigen 15-3(CA15-3)and carcinoembryonic antigen(CEA)levels and clinicopathological parameters in pa-tients with metastatic breast cancer(MBC).Methods Data of 284 MBC patients treated in the First Affiliated Hospital of Wannan Medical College between January 2007 and December 2012 were collected.Serum CA15-3 and CEA levels were measured when they were diagnosed as having MBC.The relationships between serum CA15-3 and CEA levels and clinicopathological parame-ters were analyzed.Results Of the 284 patients,elevated serum CA15-3 and CEA levels were i-dentified in 163(57.4%)and 97(34.2%),respectively.The elevated serum CA15-3 and CEA lev-els were significantly associated with breast cancer molecular subtypes(P 〈0.05).The percentage of patients with elevated serum CA15-3 and CEA levels among patients with luminal subtypes was higher than that among patients with HER2-enriched and TN subtypes.The elevated serum CA15-3 levels were correlated with bone metastasis(P 〈0.05).However,the elevated serum CEA levels were not correlated with the sites of metastasis.The percentage of patients with elevated serum CA15-3 levels among patients with multiple metastases was higher than that among pa-tients with single metastasis(P 〈0.05).There was no significant difference in the incidence of se-rum CEA elevation between patients with single metastasis and patients with multiple metastases (P 〉0.05).Conclusion The elevated CA15-3 and CEA levels are associated with breast cancer molecular subtypes in patients with MBC.The percentage of patients with elevated serum CA15-3 levels among patients with bone metastasis is higher than that among patients with other metasta-sis,and that among patients with multiple metastases is higher than that among patients with sin-gle metastasis.The elevated CEA levels are not correlated with the sites of metastases.
