摘  要:目的研究牙体磨耗至颌间高度降低对颞下颌关节(TMJ)负荷的影响。方法在已建立的人TMJ负荷分析模型上,记录并分析不同颌间高度咬合时的双侧TMJ上下腔关节面压力分布情况。结果 (1)不同颌间高度降低时,双侧TMJ负荷的力度中心均往右侧关节下腔关节面偏移,但幅度不大,各关节面压力集中区位置有不同程度的位移;(2)随着颌间高度的降低,各关节面压力峰值均逐渐增加(F左上腔=23.90、F左下腔=18.55、F右上腔=24.36、F右下腔=33.14,P均<0.05),下腔关节面均比上腔高;(3)随着颌间高度的降低,各关节面压力接触面积变化无统计学意义(F左上腔=0.052,P左上腔=0.95;F左下腔=0.122,P左下腔=0.89;F右上腔=0.312,P右上腔=0.74;F右下腔=0.097,P右下腔=0.91)。结论牙体磨耗至颌间高度降低可造成关节结构之间的相对位移,改变其内在受力环境,最终可能引发其生物力学改建。Objective To analyze the temporomandibular joint (TMJ) loading affected by the decreasing interarch distance with occlusal attrition . Methods With the model reconstructed in preliminary study, record and analyze the loading distribution of TMJ when the interarch distance was decrease. Results (1)In the different interarch distance, the stress center of TMJ shifted to lower interface of the right side, and the most pressed area of the articular surfaces was displacement inordinately . (2) As the interarch distance decreased, all the interface peak pressure of TMJ increased (P〈0.05), and the interface peak pressure in lower compartment of TMJ is greater than the upper (P〈0.05). (3)As the interarch distance decreased, the contact area of the TMJ have no significant different. Conclusions When the the interarch distance decreased because of the the dental attrition , the occlusion relationship would be changed, which caused the joint structure had a relative displacement and the change of its internal stress environment. All could eventually cause the biomechanical reconstruction.
