透明角膜活瓣切口术后眼内炎与眼内压关系研究

摘　　要：目的 探讨3.2 mm无缝线透明角膜活瓣切口术后眼内炎与眼内压的关系.方法 60只白兔60只有眼按数字表法随机分为高眼压组（27只眼,手术结束时控制眼内压20～25 mmHg）、低眼压组（27只眼,手术结束时控制眼内压10～15 mmHg）和空白对照组（6只眼,角膜未作切口）.术后1min结膜囊内滴入荧光素钠（0.2 g/1ml） 0.1 ml.分别于术后12 h、24 h测定兔眼前房水荧光素钠浓度.结果 低眼压组术后12 h、24 h前房水荧光素钠浓度显著高于高眼压组,差异有统计学意义（P＜0.05）.低眼压组和高眼压组术后12h、24 h眼压与术前眼压,差异无统计学意义（P＞0.05）.结论 术后短期内适度高眼压可促进透明角膜活瓣切口的自行闭合,防止结膜囊内液体通过切口倒流回前房,从而降低眼内炎发生的风险.Objective To investigate the relationship between postoperative intraocular pressure and endophthalmitis after 3.2mm sutureless clear corneal incision in rabbit eyes.Methods 3.2mm sutureless clear corneal incision was performed on 60 rabbits.Sixty rabbits （60 eyes） were randomly divided into 3 groups：high intraocular pressure （IOP） group （n =27,IOP set at 20-25mmHg）,low IOP group （n =27,IOP set at 10-15mmHg） and blank control group （n =6,no incision）.Each eye in experiment groups received 3.2mm sutureless clear corneal incision.At the end of the operation,the IOP was set at 20 to 25 mmHg in high IOP group while at l0 to 15 mmHg in low IOP group.One minute later,0.1ml fluorescein （FINa） （0.2g/1ml） was instilled into the conjunctival sac.The eyes in blank control group were left untreated.The FINa concentrations were measured at postoperative 12 hours and 24 hours respectively.Results The FINa concentrations in aqueous humor of low IOP group were higher than that of high IOP group at both 12 hours and 24 hours after operation,and there was a significant difference （P ＜0.05）.There was no significant difference in IOP between the blank control group and experimental groups at both 12 hours and 24 hours after operation.Conclusions The short-term,moderate IOP elevation can reduce the risk of postoperative endophalmitis,facilitating self closure of the incision and preventing aqueous backflow via 3.2mm sutureless valve clear corneal incision.
