CT 引导下微波消融联合吉西他滨与顺铂方案化疗治疗中晚期肺鳞癌临床观察

摘　　要：目的：探讨 CT 引导下微波消融联合吉西他滨与顺铂方案化疗治疗中晚期周围型肺鳞癌的疗效和安全性。方法选择2012年2月至2014年2月中晚期周围型肺鳞癌患者80例，采用随机数字表法，按不同治疗方案分为观察组和对照组，每组40例。观察组采用 CT 引导经皮冷循环微波消融联合4周期吉西他滨、顺铂方案化疗；对照组单纯采用吉西他滨、顺铂方案化疗，共4个周期。治疗结束后随访4-24个月，评价两组的疗效及不良反应。结果观察组与对照组比较，近期疗效及无进展生存期（PFS）差异有统计学意义（P 〈0.05），不良反应及并发症可耐受。结论 CT 引导下微波消融联合吉西他滨与顺铂方案化疗治疗中晚期肺鳞癌安全有效，可明显延长患者无进展生存期，改善患者的生活质量。Objective To investigate the clinical effect and safety of microwave ablation with water circulation combined with chemotherapy for treating middle-late stage lung squamous cell carcino-ma. Methods According to the different treatment plans,a total of 80 patients with middle-late stage lung squamous cell carcinoma,from February 2012 to February 2014,were randomly divided into control group and observation group,with 40 cases in each group. The patients in observation group received CT guided microwave ablation with water circulation cooling system,then received four cycles of gemcitabine and cisplatin intravenous chemotherapy;the patients in control group received only four cycles of gemcit-abine and cisplatin intravenous chemotherapy. Patients were followed up for 4 - 24 months,and the clini-cal effects and the toxicities were analyzed between the two groups. Results Compared with the control group,the PFS and short-term effect of the observation group had obvious statistical significance（P 〈0. 05）. And there was no significant difference between the two groups in the toxicity or side effects. Conclusions CT guided microwave ablation with water circulation combined with chemotherapy for trea-ting middle-late stage lung squamous cell carcinoma is effective and safe. It can prolong the PFS and im-prove their quality life.
