摘  要:目的探讨Ghrelin在反流性食管炎(reflux esophagitis,RE)中的表达水平及意义。方法选取RE患者32例作为实验组,无胃食管反流病(GERD)症状、胃镜下无RE表现的患者15例作为对照组。用快速尿素酶试验和血清抗体检测所有患者H.pylori均为阴性。收集每个研究对象的血清和胃黏膜组织标本,采用免疫组化检测胃黏膜中Grhelin的表达水平,放射免疫分析法(RIA)测定血浆Ghrelin浓度。结果实验组患者胃黏膜Ghrelin表达显著低于对照组(393.52±49.29 vs 430.93±47.53,P<0.05);实验组血浆浓度亦明显低于对照组[(1.48±0.98)ng/ml vs(2.16±1.23)ng/ml,P<0.05]。结论随着Ghrelin水平的降低,可导致胃排空延缓,使胃内压升高,提高一过性食管下括约肌松弛(TLESR)频率,从而可能在RE发病机制中起一定作用。Objective To investigate the expression and clinical significance of Ghrelin in reflux esophagitis (RE).Methods Thirty-two patients with RE were selected as the experiment group and 15 patients with no gastroesophageal reflux diease (GERD) symptoms and esophageal mucosa lesion as the control group.Detection of H.pylori were negative by rapid urease test and serum antibody test.The Ghrelin expression levels of mucosal tissue which taken from gastric fundus were examined by immunohistochemical staining.The plasma Ghrelin protein expression levels were examined by radioimmunoassay (RIA).Results The expression levels of Ghrelin were significantly decreased in experiment group compared with control group (393.52± 49.29 vs 430.93 ± 47.53,P < 0.05).Plasma concentration of Ghrelin levels were significant decreased in experiment group compared with control group [(1.48 ± 0.98) ng/ml vs (2.16 ± 1.23)ng/ml,P < 0.05].Conclusion Decreased Ghrelin levels maybe delay gastric emptying and lead to elevating of the intragastric pressure,which can increase the frequency of transient lower esophageal sphincter relaxation (TLESR),and may play an important role in the pathogenesis of RE.
