摘 要:目的:本文旨在阐明从大鼠心包注入炎性渗出液(IES)与大鼠三叉神经脊束核(STT)神经元激活的相关性。方法:成年雄性Wistar大鼠用氨基甲酸乙酯麻醉后随机分为3组:从心包腔注入炎性渗出液为IES组;生理盐水组(saline组)大鼠心包腔注入等量的生理盐水,其余手术过程同IES组;假手术组(sham组)大鼠心包腔内既不注射IES也不注射生理盐水,手术程序同IES组。运用免疫组化方法,以神经元活化为线索,观察大鼠脊髓和脑干神经元c-Fos表达,特别关注大鼠脑干三叉神经感觉核复合体神经元c-Fos的表达,包括三叉神经感觉主核腹外侧部分(Pr5VL)以及三叉神经脊束核口部(Sp50)、极间部(Sp5I)和尾部(Sp5C)。结果:同saline组和sham组相比,IES组大鼠C2段脊髓背角Ⅰ-Ⅴ层c-Fos阳性神经元数量显着增加(P<0.05),三叉神经脊束核Sp5C、Sp50、Sp5I及Pr5VL腹侧边c-Fos阳性神经元数量也明显增加(P<0.05),呈圆锥体状的三叉神经感觉核复合体c-Fos阳性神经元的数量自前囟到对耳线逐渐增加,c-Fos阳性神经元的数量右侧少于左侧(P<0.05)。Sp5C腹侧边c-Fos阳性神经元主要集中在胶质细胞层,大细胞层较少(P<0.05)。用育亨宾(1 mg/kg,ip)和纳络酮(3 mg/kg,ip)预处理20 min,IES组大鼠Sp5C腹侧边c-Fos阳性神经元的表达未受影响。结论:向心包腔注入致痛物质模拟大鼠心肌缺血心绞痛,可以活化三叉神经脊束核神经元。这为临床上部分心肌缺血患者颅面部疼痛是其唯一的症状提供了实验依据。AIM: To identify the correlation between intrapericardial injection of allogeneic chemicals and activation of the rat spinal trigeminal tract( STT) neurons. METHODS: Adult male Wistar rats were anesthetized with urethane and divided into 3 groups: inflammatory exudate solution( IES) group,receiving intrapericardial infusion of IES;negative control( saline) group,receiving intrapericardial infusion of saline; and sham group,undergoing the same surgical procedure without intrapericardial infusion. The expression of c-Fos in the neurons of the cervical spinal cord and brain stem,particularly in the region of the trigeminal sensory nuclear complex,including the principal sensory trigeminal nucleus( Pr5VL),subnucleus oralis( Sp5O),subnucleus interpolaris( Sp5I) and subnucleus caudalis( Sp5C),was analyzed by immunohistochemistry as an index of neuronal activation. RESULTS: Compared with saline group and sham group,the number of c-Fos-positive neurons in IES group increased significantly( P〈0. 05). c-Fos-positive neurons were mainly found in the dorsal horns of laminae I-V at C2,as well as at the rostral edge of the Pr5 VL,Sp5O,Sp5 I,and Sp5 C.The number of c-Fos-positive neurons gradually increased from bregma to interaural line at the pyramidal decussation. Interestingly,fewer c-Fos-positive neurons were observed on the right side than that on the left( P〈0. 05). CONCLUSION: Intrapericardial injection of algogenic chemicals activates STT neurons,possibly underlying the phenomenon whereby cardiac ischemia causes craniofacial pain.
