摘　　要：目的探讨MSCT对成人纵隔淋巴结结核与纵隔淋巴瘤的鉴别诊断价值。方法回顾性分析30例成人纵隔淋巴结结核及26例纵隔淋巴瘤的MSCT图像资料。结果两组病例累及的部位不具有鉴别意义,但累及的部位数区别较大,纵隔淋巴瘤累及的部位更为广泛。淋巴结大小、形态对两组病变无鉴别意义,而纵隔淋巴瘤组中77%病例出现融合,具有一定的特征性。两组病例在密度及增强方式上区别较大,成人纵隔淋巴结结核平扫密度易出现钙化、坏死,增强后以环形强化多见;而淋巴瘤平扫密度均匀,增强后以均匀强化多见,此特征具有鉴别意义。结论胸部MSCT扫描密度分辨率和空间分辨率高,能够较好地显示纵隔结构,增强CT对肿大淋巴结的分布、形态、大小、边缘、密度显示清晰,MSCT对成人纵隔淋巴结结核与纵隔淋巴瘤的诊断及鉴别诊断具有重要的意义。Objective To investigate the value of MSCT in differential diagnosis of mediastinal tuberculous lymphadeni -tis in adults and mediastinal lymphoma .Methods Image data of 30 cases of mediastinal tuberculous lymphadenitis in adults and 26 cases of mediastinal lymphoma were retrospectively analyzed .Results Nodal station of the 2 groups had no difference ,but the number of the nodal station was different .Nodal station of mediastinal lymphoma was more extensive .Lymph node size and shape had no differential significance between the 2 groups.Fusion of the lymph nodes was seen in most of mediastinal lymphomas cases （77%）,there had obvious characteristics.There were significant difference in density and CT enhancement between the 2 groups.Mediastinal tuberculous lymphadenitis in adults were more prone to calcification and necrosis ,and mostly show periphery enhancement;Density in plain CT scan and enhancement are homogeneous in most of mediastinal lymphomas ,there had differenti-al significance .Conclusion The chest MSCT can show clearly mediastinal structures with high density resolution and spatial res -olution.The distribution,modality,size,verge and density can be displayed clearly .The distinction is obvious in density and CT enhancement patterns about these 2 groups .MSCT has significant value in differential diagnosis of mediastinal tuberculous lym -phadenitis in adults and mediastinal lymphoma .
