力尔凡膀胱灌注预防浅表性膀胱尿路上皮癌术后复发临床研究

摘　　要：目的探讨力尔凡膀胱灌注预防浅表性膀胱尿路上皮癌术后复发。方法将152例浅表性膀胱尿路上皮癌术后患者随机分成两组。试验组:力尔凡+DDP+放疗;对照组:DDP+放疗。比较两组复发率、复发间隔时间,观察治疗前后T细胞亚群和白细胞总数。结果不同手术方式的复发率、复发间隔时间差异不显著(P>0.05);而试验组与对照组复发率、复发间隔时间比较均有显著性差异(P<0.05及P<0.01);试验组治疗前后NK细胞及CD4/CD8值比较有显著性差异(P<0.01),治疗后明显高于治疗前,对照组则无显著性差异;试验组治疗前后白细胞总数比较无显著性差异(P>0.05),对照组则有显著性差异(P<0.01),治疗后明显低于治疗前。结论力尔凡不但可以杀灭肿瘤细胞,而且有免疫复苏作用,在体内与其他免疫因子和化疗药物起协同作用,对膀胱尿路上皮癌不失为1种化疗配伍的安全、有效的药物。Objective To evaluate intravesical Lifein instillation in prophylaxis of postoperative recurrence of superficial bladder urothelial carcinoma .Methods 152 cases of superficial bladder urothelial carcinoma patients after operation were ran -domized into the experimental group （ intravesical Lifein instillation ＋DDP ＋Radiotherapy ） and the control group （ DDP ＋Radiotherapy ） .Recurrence rate ,recurrence interval of the 2 groups were compared ,T cell subsets and leukocyte counts before and after treatment were observed .Results Recurrence rates and recurrence interval of different surgical approaches had no signifi -cant differences （ P〉0.05） ,but the recurrence rates and recurrence interval between the experimental group and the control group had significant differences （P〈0.05,P〈0.01 ）.NK cell and CD4/CD8 after treatment were significantly higher than be-fore treatment in the experimental group （ P〈0.01 ）,but there had no significant differences in the control group （ P〉0.05）. The leukocyte counts in the experimental group before and after treatment had no significant differences （ P〉0.05）,but there had significant differences in the control group （ P〈0.01 ） .Conclusion Intravesical Lifein instillation can kill tumor cells ,and is helpful for immune recovery .There is a synergistic effect among intravesical Lifein instillation ,immune factors and chemothera-py drugs in patients with superficial bladder .urothelial carcinoma .It is a safe and effective compatibility of medicines for chemo-therapy in superficial bladder urothelial carcinoma .
