急性缺血性卒中患者脑微出血的危险因素研究

摘　　要：目的探讨急性缺血性卒中患者脑微出血(cerebral microbleeds,CMBs)的相关危险因素。方法选择2012年11月-2013年6月于宁夏医科大学总医院神经内科住院的急性缺血性卒中患者共82例,应用磁敏感加权成像技术(susceptibility weighted imaging,SWI)检测CMBs,根据检查结果,将82例患者分为病例组(有CMBs者,40例)和对照组(无CMBs者,42例),比较两组间实验室检查及影像学特点是否存在差异。结果急性缺血性卒中患者发生CMBs与高血压(χ2=11.923,P=0.001)、脑白质疏松(χ2=9.694,P=0.002)、腔隙性脑梗死(χ2=5.567,P=0.018)之间有显著相关性;Logistic回归分析显示,脑白质疏松(OR=6.640,P=0.001)、腔隙性脑梗死(OR=8.155,P<0.001),可能是急性缺血性卒中患者发生CMBs的独立危险因素。结论高血压、脑白质疏松、腔隙性脑梗死可能是急性缺血性卒中患者发生CMBs的危险因素。Objective To investigate the risk factors of cerebral microbleeds （CMBs） in patients with acute is- chemic stroke; Methods 82 patients with ischemic stroke who were hospitalized in department of neurology of general hospital of Ningxia medical university from november 2012 to June 2013 were successionally recrui- ted. Susceptibility weighted imaging（SWI） was adopted for CMBs. 82 cases were divided into the experimen- tal group （CMBs group,40 cases） and the control group （non- CMBs group,42 cases） based on the pres- ence of CMBs. The difference of clinical data including general conditions,laborartory test parameters and im- aging features between the two groups was compared. Results CMBs were significantly related to hyperten-sion（X2 = 11. 923, P =0.001 ）, leukoaralosls （X2 = 9. 694, P = 0. 002 ）, lacunar infarction （ X2 = 5. 567, P = 0. 018）in patients with acute ischemic stroke. Logistic regression analysis showed that leukoaraiosis （OR = 6. 640, P =0. 001 ） ,lacunar infarction（ OR = 8. 155, P 〈 0.00 l ） were the independent risk factors for presence of CMBs. Conclusion CMBs had relationship with hypertension, [eukoaraiosis and lacunar infarction~
