经尿道绿激光汽化术治疗非肌层浸润性膀胱肿瘤的临床分析

摘　　要：目的探讨选择性绿激光治疗非肌层浸润性膀胱癌的手术技巧及疗效。方法回顾分析应用经尿道选择性绿激光治疗7例非肌层浸润性膀胱癌患者的临床资料。结果所有手术均成功,手术时间平均17.5min。术中出血少,无膀胱穿孔,无闭孔神经反射等严重并发症。术后膀胱冲洗2-6h。单纯膀胱肿瘤术后导尿管保留48h,一期行前列腺汽化者导尿管保留72h。术后应用吡柔比星膀胱灌注,随访3个月至1年,肿瘤无复发。结论绿激光汽化术治疗膀胱肿瘤具有手术时间短、出血少、并发症少的特点,是治疗非肌层浸润性膀胱肿瘤的一种安全、有效的方式。Objective To evaluate the efficacy and technique of transurethral photoselective green laser vaporization for the treatment of non-invasive bladder tumor. Methods Clinical data of 7 patients used transurethral photoselective green laser vaporization in treating their non-invasive bladder tumor were retrospectively analysed. Results All the operations were performed successfully.The mean time of operation was 17.5minutes.There was no lots of bleeding,bladder perforation,obturator nerve reflex,and other serious complication.The time of postoperative bladder washing was 2-6hours.The time of postoperative catheterization for patients with simple bladder tumor and stage 1tumor was 48 hours and 72 hours respectively.All patients received postoperative bladder irrigation with pirarubicin and followed up for 3month to 1year.There was no recurrence of tumor. Conclusions Transurethral photoselective green laser vaporization has the advantages of short operation time,less bleeding,and less complication.It＇s a safe and efficient treating method to non-invasive bladder tumor.
