B7-H1、B7-H3、B7-H4在胃癌中的表达及其临床意义

摘　　要：目的 探讨B7-H1、B7-H3及B7-H4在胃癌中的表达及其临床意义.方法 应用免疫组织化学的方法,检测85例胃癌患者手术标本B7-H1、B7-H3、B7-H4的表达,以及它们与各项临床病理特征之间的关系.结果 B7-H1在胃癌患者中的阳性率为68.6％（59/85）,其表达与年龄、性别、肿瘤浸润深度、远处转移、分化程度、TNM分期均无明显相关,但与淋巴结转移明显相关（P＜0.05）,淋巴结转移组的B7-H1的阳性率高于未转移组（χ2=8.594,P＜0.01）.B7-H3在胃癌患者中的表达率为5.9％ （5/85）,B7-H4的表达率为41.1％ （30/85）.B7-H3、B7-H4的表达与年龄、性别、肿瘤浸润深度、淋巴结转移、远处转移、分化程度、TNM分期等临床病理特征均无明显相关（P＞0.05）.结论 胃癌表达B7-H1水平与淋巴结的转移明显相关,表明B7-H1在胃癌的淋巴结转移有重要作用.Objective To investigate the expression of three members of B7 family,B7-H1,B7-H3 and B7-H4 in gastric carcinoma tissues,and explore the correlation between their expression levels with clinicopathological features.Methods The expression of three co-stimulatory molecules in tumor sections from 85 patients with gastric cancer was detected by using immunohistochemistry.The correlation between the expression levels with clinicopathological features was analyzed by chi-square text or Fisher＇ s exact test.Results The positive expression rate of B7-H1 was 68.6% （59/85） in gastric carcinoma.The B7-H1 expression was significantly correlated with lymph node metastasis （χ2 =8.594,P 〈0.01）,but not with other clinical features,sex,age,invasion depth,differentiation,distant metastasis and TNM stage.The high expression rate of B7-H3 and B7-H4 in tumor tissues was 5.9% （5/85） and 41.1% （30/85）,respectively.The relationship between the expression of two co-stimulatory molecules with all clinical features was not demonstrated in the study.Conclusion There was a positive correlation between the B7-H1 expression and lymph node metastasis.The finding suggests that B7-H1 may plays a key role in lymph node metastasis.
