幽门螺杆菌阳性复发性消化性溃疡三联及四联用药的临床疗效分析

摘　　要：为比较四联疗法与三联疗法治疗幽门螺杆菌(HP)阳性复发性消化性溃疡的临床疗效,将169例经14C-尿素呼气试验及胃镜诊断证实HP阳性的复发性消化性溃疡患者随机分为2组。A组89例:兰索拉唑60 mg/d+阿莫西林2.0 g/d+克拉霉素1.0 g/d,共14 d;B组80例:兰索拉唑60 mg/d+阿莫西林2.0 g/d+克拉霉素1.0 g/d+枸盐酸铋钾480 mg/d,共14 d。治疗后复查14C-尿素呼气试验及胃镜检查,检测HP根除率及溃疡愈合情况,并观察症状缓解时间。结果为:四联疗法组与三联疗法组HP根除率分别为92.5%,77.53%,溃疡愈合率分别为98.75%,88.76%,差异有统计学意义(P<0.01);且不同类型溃疡的症状缓解时间四联疗法均短于三联疗法,存在统计学差异(P<0.05)。四联疗法治疗HP阳性复发性消化性溃疡优于三联疗法。Objective ：To compared the effect of four- drug treatment with that of three -drug treatment. Both of two treatments were used to treat helicobacter pylori（HP） positive recurrent ulcer. Methods 169 cases that had been diagnosed as HP positive recurrent ulcer by14C breath test and gastroscope were divided into two groups randomly. Group A contained 89 cases ： Lansoprazole 30mg/d ＋ Amoxicillin 2.0g/d ＋ Clarithromycin 1.0 g/d, the total was 14 d ; Group B contained 80 cases ： Lansoprazole 30 mg/d ＋ Amoxicillin 2.0 g/d ＋ Cla- rithromycin l. 0 g/d ＋ hydrochloric acid and bismuth potassium citrate 480 rag/d, the total is 14 d. After re- ceiving the treatment, two groups were re - examined through14C breath test and gastroscope in order to check eradication rate of HP and the recovery of ulcer and observe the symptom remission time. Result ： The eradica- tion rates of four - drug group and three - drug group were 92.5% and 77.53% respectively,and the recov- ery rates of them were 98.75% and 88.76% respectively. The differences were significant（P 〈0.01 ） ;And the symptom remission time of Four - drug treatment was shorter than that of three - drug treatment in differ- ent types of ulcer, there was statistical difference （ P 〈 0. 05 ）. Conclusion ： The four - drug treatment that trea- ted HP positive recurrent ulcer was better than that of the three - drug treatment.
