乳腺叶状肿瘤与纤维腺瘤超声图像比较分析

摘　　要：目的探讨乳腺叶状肿瘤(PTB)与纤维腺瘤(BF)超声图像的共同点及差别,为提高PTB的诊断率提供依据。方法选择76例PTB患者及172例BF患者的临床资料,回顾性分析PTB及BF的临床特征及超声特征。结果 PTB组患者的年龄明显高于BF组(P<0.05);PTB组患者肿块直径明显大于BF组(P<0.05)。PTB组患者肿块以分叶状、包膜较完整、内部回声均匀、后方回声增强或不变、有囊性变、血流丰富程度达Ⅰ级以上的比例明显高于BF组(P<0.05),两组的钙化情况差异不明显(P>0.05)。患者的年龄、肿块直径、形态、包膜、内部回声、后方回声、囊性变及血流分级是PTB的独立危险因素。结论超声在PTB和BF的鉴别诊断中具有许多特异性指标,如PTB肿物呈分叶状,边界清晰,周围皮肤变薄;肿物内部有囊性无回声区;肿物内部回声不均匀;肿物内部血流丰富,静脉曲张明显。超声检查的特异性图像特征对PTB的术前诊断有较高的价值,可为选择合理的手术方式提供依据。Objective To investigate the similarities and differences of ultrasound image between breast phyllodes tumor （ phyllodes tumor of the breast, PTB） and fibroadenoma （breast fibroadenoma, BF）, to provide basis for improving diagnostic rate in PTB. Methods The clinical data of 76 patients with PTB and 172 patients with BF were selected. The clinical characteristics and ultrasonic features between PTB and BF were analyzed. Results The age in PTB group were significantly higher than that in BF group （ P 〈 0.05 ） ; the tumor diameter in PTB group was larger than that in BF group （P 〈 0. 05）. In PTB group the proportion of patients＇ tumor were most phyllodes, its capsule were complete, its echo was uniform, its posterior acoustic was enhanced or unchanged, and it is with cystic change, rich blood flow reached grade I was higher than that in BF group （P 〈 0. 05 ）. The calcification between two groups was not significantly different （P 〉 0. 05）. The patients＇ age, tumor diameter, shape, envelope, internal echo, rear echo, cystic degeneration and flow classification were independent risk factors of PTB. Conclusion There are many specific indices in differential diagnosis about PTB and BF by ultrasound： such that tumor is lobulated, clear boundary, surrounding skin is thinning; tumor internal anechoic is cystic; tumor internal echo is not uniform; tumor blood is perfusion, and varicose veins are obvious. The specificity of ultrasound image characteristics has rela- tively high value, and can provide reference for rational operation.
