慢性胰腺炎与肿瘤坏死因子α基因308A/G多态性风险关联的meta分析

摘　　要：目的采用meta分析探究肿瘤坏死因子α（TNF-α）基因308A/G多态性与慢性胰腺炎（CP）之间的关联性。方法系统检索电子数据库Pub Med、Embase、Web of Science、中国知网全文数据库（CNKI）、中国生物医学文献数据库（CBM）和万方数据库等。对CP患病风险与TNF-α基因308A/G多态性的病例对照研究数据处理后,使用比数比（OR）以及95%可信区间（95%CI）对其关联性进行meta分析,对发表偏倚进行检验。结果检索到可能相关的研究798项,通过阅读标题、摘要以及全文,最后有8项相关研究进入最后的meta分析,包括CP患者659例和对照者742例。最终,把所有的研究纳入到meta分析后,并没有发现TNF-α基因308A/G多态性与CP之间的关联（A vs.G：OR=1.14,95%CI=0.93～1.40,P=0.21;AA vs.AG＋GG：OR=1.63,95%CI=0.99～2.67,P=0.24;AA vs.GG：OR=1.55,95%CI=0.98～2.45,P=0.56;AA＋AG vs.GG：OR=1.03,95%CI=0.81～1.32,P=0.76）。除此之外,在对种族进行亚组分析时,也得到相似的结果。结论 meta分析显示,CP与TNF-α基因308A/G多态性之间没有关联性。但是,由于样本量较小以及部分研究选择偏倚因素的存在,对结论的解释理解要慎重。Objective A meta-analysis is adopted to explore the revelence between tumor necrosis factor-alpha（TNF-α）gene-308A/G polymorphism and chronic pancreatitis（CP）. Methods It was used systematic searches of electronic databases such as Pub Med,Embase,Web of Science,CNKI,CBMs and Wanfang data. All the case-control studies investigating the association between TNF-α gene-308A/G polymorphism and CP risk were included. The association was assessed by odds ratio（OR） with 95%confidence intervals（95%CI）. Publication bias was examined. Results A total of 798 studies potentially related to it were found.Having read the title,abstracts,or full texts,8 relevant studies were included in the final meta-analysis,including 659 CP patients and742 cases as the control group. Ultimately,it found out that there was no significant association between TNF-α gene-308A/G polymor phism and CP risk when all the studies were pooled into the meta-analysis（for A vs G：OR=1.14,95%CI=0.93-1.40,P=0.21;AA vs AG＋GG：OR=1.63,95%CI=0.99-2.67,P=0.24;AA vs GG：OR=1.55,95%CI=0.98-2.45,P=0.56;AA＋AG vs GG：OR =1.03,95%CI=0.81-1.32,P =0.76）. Besides,the similar results were obtained in the subgroup analysis based on the ethnicity. Conclusion The meta-analysis reveals that the TNF-α gene-308A/G polymorphism is not associated with CP. However,due to the small numbers of samples and the selection bias is included in this study,the results should be interpreted with cautious attitude.
